Methandrostenolone

Dianabol / Dbol (metandienone, methandienone); oral 17aa AAS

Evidence grade 2 of 5, Thin. Read how the Atlas grades evidence.

The written rationale for this grade is still being prepared.

FDA
Former US approval long withdrawn; Ciba ceased production; Schedule III
WADA
Prohibited at all times (S1 Anabolic Agents).

This is something to take to a clinician. The Atlas can tell you what is known about methandrostenolone in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.

Methandrostenolone (Dianabol) is an oral 17-alpha-alkylated testosterone derivative, testosterone with an added 1,2 double bond and a 17-alpha-methyl group. Synthesized by Ciba and introduced in 1958, it became the first widely used performance steroid in the US after physician John Ziegler introduced it to weightlifters around 1959-1960, reportedly in response to Soviet androgen use. It aromatizes (estrogenic effects, water retention, gynecomastia) and is hepatotoxic as a 17aa oral. Ciba eventually ceased production and US approval lapsed; it persists almost entirely through illicit manufacture.

The original performance steroid, born from the Cold War of weightlifting.

What people take it for

These are the reasons people give for taking it. They are not effects it has been shown to have.

  • Historical minor medical use (debility, osteoporosis-era trials)
  • Illicit physique/strength use (a classic 'bulking' oral)
Historical debility/osteoporosis-era use
Limited mid-century medical use before its risk profile and the rise of alternatives ended legitimate prescribing.

How it is supposed to work

Methandrostenolone binds the androgen receptor to drive protein synthesis, glycogen retention and nitrogen balance, producing rapid scale-weight gains (substantially water). The 1,2 double bond and 17-alpha-methyl group give oral activity and slowed aromatization, but it still converts to an estrogen, causing fluid retention and gynecomastia risk.

Testosterone with a double bond between carbons 1 and 2 and a 17-alpha-methyl group (the 17aa modification enabling oral dosing and conferring hepatotoxicity).

Fast gains, mostly water, and a liver cost

Much of Dianabol's rapid early 'gains' is intracellular water and glycogen, which dissipates on discontinuation. Meanwhile, as a 17aa oral it raises liver enzymes and can cause cholestasis and, with prolonged use, serious hepatic lesions, while suppressing HDL and the HPTA. The dramatic short-term scale change masks a poor risk profile.

Safety in detail

This intervention involves significant physiological effects and requires evaluation and monitoring by a qualified physician. Self-administration without medical oversight carries documented health risks.

  • 17aa hepatotoxicity: elevated transaminases, cholestasis, peliosis hepatis with prolonged use.
  • Estrogenic effects: water retention, hypertension, gynecomastia.
  • Marked HDL suppression.
  • HPTA suppression.
  • Illicit-only supply with attendant counterfeit/contamination risk.

Replacement is not the same as a cycle

Any present-day use is supraphysiological and unsanctioned; there is no legitimate US product. This entry documents history and harm, not dosing.

What it does to fertility

Suppresses LH/FSH and spermatogenesis; subfertility during use with variable recovery afterward.

What has to be monitored

The bloodwork this needs, and the reason it belongs with a clinician rather than a website.

  • Liver function
  • Lipid panel
  • Blood pressure
  • Estradiol
  • LH/FSH, testosterone
  • Hematocrit

What people get wrong

  • Often said'Dbol is the best way to put on real muscle fast'

ActuallyMuch of the early gain is water/glycogen that disappears after discontinuation; the trade-off is hepatotoxicity, estrogenic side effects, lipid harm and suppression.

  • Often said'Ziegler invented Dianabol'

ActuallyCiba synthesized and introduced methandrostenolone (1958); Ziegler popularized its athletic use in the US around 1959-1960 and later publicly regretted it.

Who profits, and who pushes back

Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.

How we got here

Methandrostenolone was synthesized and introduced by Ciba in 1958. Dr. John Ziegler, team physician to US weightlifters and a fixture at Bob Hoffman's York Barbell Club, introduced it to American lifters around 1959-1960, the founding episode of organized performance-steroid use in the West. Reports of negligible gains at the 1960 Olympics aside, its use spread rapidly through strength sport in the 1960s-70s. Ziegler later disavowed it; Ciba eventually discontinued production and US approval lapsed.

What this entry does not yet cover

What follows is unfinished work on our side, not a finding about Methandrostenolone. A short entry is not a safe entry.

  • Cost and market. We have not documented what it sells for or who sells it.
  • Supply chain. We have not mapped where it is made.
  • Who uses it. We have not documented the user groups.
  • What could change. No forward-looking summary yet.
  • How it compares. We have not set it against the alternatives.
  • Children and teenagers. We have not checked what is known about younger people.
  • What can go wrong. We have not written the risk line for this one. Read the safety section below rather than treating its absence as reassurance.
  • Where we land. We have not taken a position on this yet.
