This is something to take to a clinician. The Atlas can tell you what is known about prolactinoma & pituitary causes in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
A prolactinoma is a benign pituitary tumor that oversecretes prolactin. Elevated prolactin suppresses hypothalamic GnRH pulses, partly by inhibiting kisspeptin-secreting neurons, which lowers LH and FSH and therefore testosterone, producing a secondary hypogonadism. In men it often presents with low libido, erectile dysfunction and, with larger tumors, headaches or visual field loss. It is one of the more reversible causes of low testosterone because dopamine agonists usually shrink the tumor and restore the axis.
A small benign tumor that switches off the testosterone signal.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
Lactotroph tumor cells autonomously secrete prolactin. Excess prolactin acts on the hypothalamus to blunt pulsatile GnRH release, in part by suppressing kisspeptin neurons that drive GnRH. Lower GnRH means lower LH and FSH, so the testes receive less stimulation and testosterone falls, a hypogonadotropic pattern. Dopamine normally restrains prolactin secretion via negative feedback; dopamine agonists exploit this, lowering prolactin and often shrinking the tumor.
The test that is easy to skip and costly to miss
Checking prolactin is a cheap test that is easy to skip in a routine low-T workup, yet missing a prolactinoma means missing both a treatable cause of low testosterone AND a pituitary tumor that can threaten vision. Putting such a man on testosterone without measuring prolactin can mask the tumor's hormonal fingerprint while the mass keeps growing.
Safety in detail
Prolactinoma involves a pituitary tumor and requires endocrine and sometimes neurosurgical evaluation, plus pituitary imaging. Treating the low testosterone symptom without diagnosing the underlying tumor can delay care for a mass that may threaten vision. Dopamine agonists require physician supervision.
What it does to fertility
Hyperprolactinemia impairs spermatogenesis along with testosterone, so fertility can be affected. Because the suppression is functional, normalizing prolactin with a dopamine agonist commonly restores both testosterone and sperm production, making this one of the more fertility-friendly causes of low testosterone to correct.
What has to be monitored
The bloodwork this needs, and the reason it belongs with a clinician rather than a website.
- Serum prolactin
- LH, FSH, total testosterone
- Pituitary MRI for macroadenomas
- Visual fields if tumor abuts the optic chiasm
What people get wrong
Often saidLow testosterone with normal LH just needs a testosterone prescription
ActuallyNot before checking prolactin. A prolactinoma produces exactly this pattern, is treatable, and can be a growing tumor, testosterone alone would treat the symptom and hide the cause.
Often saidPituitary tumors always need surgery
ActuallyMost prolactinomas respond to dopamine agonists, which frequently shrink the tumor and restore testosterone without an operation.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
At a glance
- Mechanism
- High prolactin suppresses GnRH via kisspeptin inhibition
- First-line therapy
- Dopamine agonists (cabergoline, bromocriptine)
- Tumor effect
- Cabergoline typically shrinks tumor and restores gonadal function
