What can go wrong

This is developmental biology, not an intervention, the relevant 'risk' is disruption of the cascade. Errors at any step (SRY translocation or mutation, gonadal dysgenesis, AMH/AMH-receptor defects, androgen synthesis or receptor defects, or fetal anti-androgen exposure during the masculinization window) can produce disorders of sex development, hypospadias, cryptorchidism or infertility. Findings here are reference material and not a basis for self-directed treatment.

This is something to take to a clinician. The Atlas can tell you what is known about sexual differentiation in utero in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.

Mammalian sex is determined at fertilization by the sex chromosomes, but the male phenotype is actively constructed over the first trimester through a cascade triggered by SRY, the sex-determining region on the Y chromosome cloned by Sinclair, Lovell-Badge and Goodfellow in 1990 (Nature). SRY upregulates SOX9, directing the bipotential gonad to become a testis. Fetal Sertoli cells then secrete anti-Mullerian hormone (AMH) to regress the female (Mullerian) ducts, while Leydig-cell testosterone stabilizes the Wolffian ducts into epididymis, vas deferens and seminal vesicles. Jost's classic rabbit-castration experiments (1940s-50s) established that the testis, not the ovary, is the active driver: without it, the embryo defaults toward female anatomy.

How a genetically male embryo is built, and why timing is everything.

What people take it for

What it is taken for. Approval, where it exists, covers only some of these.

  • Understanding disorders/differences of sex development (DSD)
  • Framework for AIS, 5-ARD, gonadal dysgenesis
  • Basis of the 'programming window' concept in toxicology

How it works

The gonad is bipotential until roughly week 6-7. SRY expression in pre-Sertoli cells, acting through SOX9 and a feed-forward loop with FGF9 (antagonizing the ovarian WNT4/RSPO1/beta-catenin and FOXL2 programs), commits the gonad to testis. Sertoli cells secrete AMH causing Mullerian (uterus/tubes/upper vagina) regression. Leydig cells, initially driven by placental hCG and later by fetal LH, secrete testosterone that acts locally and at high concentration to stabilize the Wolffian ducts. Internal-duct masculinization is testosterone-dependent; external-genitalia masculinization additionally requires conversion of testosterone to dihydrotestosterone by 5-alpha-reductase type 2.

Two duct systems coexist transiently in every fetus: the Wolffian (mesonephric) and Mullerian (paramesonephric) ducts. In the male, AMH eliminates the Mullerian set and androgen preserves the Wolffian set; in the female the reverse occurs by default. The external genitalia derive from common anlagen, the genital tubercle becomes glans penis or clitoris, the urethral folds fuse into the penile urethra or remain as labia minora, and the labioscrotal swellings become scrotum or labia majora.

The masculinization programming window

Work from Richard Sharpe's group (rodent models, with human inference) defined a 'masculinization programming window' (in the rat, roughly gestational days 15.5-18.5; in humans inferred to early second trimester, ~weeks 8-14) during which androgen action programs later genital and reproductive-tract outcomes. Anogenital distance (AGD) is a lifelong read-out of androgen exposure during this window. The clinical implication is that the dose-response for events like hypospadias and cryptorchidism is set early; this underlies the 'testicular dysgenesis syndrome' hypothesis linking cryptorchidism, hypospadias, poor semen quality and testis cancer to disrupted fetal androgen/Sertoli function.

What people get wrong

  • Often saidSex is simply XX or XY and fixed at conception

ActuallyChromosomes set the trajectory, but the phenotype is built by a gene cascade and hormone signaling that can diverge (SRY translocations, gonadal dysgenesis, AIS, 5-ARD), producing the spectrum studied as DSD.

  • Often saidEstrogen makes the female body

ActuallyThe female plan is largely the hormone-independent default in this era; it is the testis and its androgens/AMH that actively divert development toward male anatomy.

Who profits, and who pushes back

Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.

What we do not know

  • How precisely does the human masculinization programming window map onto gestational weeks?
  • Which environmental anti-androgens meaningfully perturb it at real-world exposures?

At a glance

Master switch
SRY gene (Yp11.3)
SRY cloned
Sinclair et al. 1990, Nature
Default pathway
Female (Jost)
Wolffian stabilizer
Testosterone (not DHT)
Mullerian regressor
AMH / MIS (Sertoli cells)

What this entry does not yet cover

What follows is unfinished work on our side, not a finding about Sexual Differentiation in Utero. A short entry is not a safe entry.

  • Timeline. We have not built the dated record of research and regulatory events.
  • Cost and market. We have not documented what it sells for or who sells it.
  • Supply chain. We have not mapped where it is made.
  • Who uses it. We have not documented the user groups.
  • What could change. No forward-looking summary yet.
  • How it compares. We have not set it against the alternatives.
  • Children and teenagers. We have not checked what is known about younger people.
  • Where we land. We have not taken a position on this yet.
