Nocturnal Penile Tumescence Testing
NPTR, distinguishing organic from psychogenic ED

What can go wrong
Non-invasive and low-risk. Limitations: REM sleep disruption (sleep disorders, depression, poor sleep) can blunt nocturnal erections and confound interpretation, so abnormal results are not perfectly specific for organic ED.
This is something to take to a clinician. The Atlas can tell you what is known about nocturnal penile tumescence testing in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Nocturnal penile tumescence and rigidity (NPTR) testing measures the spontaneous erections that occur during REM sleep, typically 3-5 per night in healthy men. Preserved nocturnal erections in a man who cannot erect when awake point to a psychogenic cause; absent or weak nocturnal erections suggest an organic (vascular, neurogenic, or hormonal) cause. The RigiScan device is the classic instrument.
Healthy men erect 3-5 times a night, measure it and you learn the cause.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
REM sleep is accompanied by reflexogenic erections independent of conscious arousal and anxiety. If the neurovascular apparatus is intact, these occur regardless of waking psychological state; their presence therefore isolates psychogenic ED, while their absence implicates an organic lesion.
What people get wrong
Often saidIf you wake with morning erections everything is fine
ActuallyMorning erections are reassuring but not a substitute for formal testing; partial or inconsistent tumescence can still accompany significant organic disease.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
