What can go wrong
The condition itself is the risk: untreated ischemic priapism beyond 24-48 hours commonly results in permanent erectile dysfunction. Men using injection therapy must be counseled on the emergency threshold and rescue plan.
This is something to take to a clinician. The Atlas can tell you what is known about priapism in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Priapism is a persistent erection lasting more than four hours, unrelated to or outlasting sexual stimulation. Ischemic (low-flow) priapism, the common, painful, dangerous form, is a compartment syndrome of the penis: trapped deoxygenated blood causes tissue ischemia and, if untreated, fibrosis and permanent ED. Causes include intracavernosal injection therapy, sickle cell disease, certain medications, and rarely PDE5 inhibitors. Non-ischemic (high-flow) priapism follows trauma and is less urgent.
An erection over 4 hours is an emergency, not a boast.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
In ischemic priapism, failure of venous outflow traps blood in the corpora; oxygen is consumed, the trapped blood becomes acidotic and hypoxic, and prolonged ischemia causes smooth-muscle necrosis and fibrosis. Treatment is corporal aspiration and intracavernosal sympathomimetic (phenylephrine) injection, escalating to surgical shunting.
Safety in detail
Priapism, an erection lasting more than 4 hours, is a urologic emergency. Delay in treatment risks permanent ischemic damage and erectile dysfunction. Immediate medical evaluation is required.
What people get wrong

ActuallyIschemic priapism is a painful emergency that damages erectile tissue. Beyond four hours it threatens permanent ED and demands immediate treatment.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
