Dapoxetine
Priligy - the only purpose-built on-demand SSRI for PE (approved abroad, not in the US)
Evidence grade 4 of 5, Strong. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- Approved in 60+ countries; FDA non-approvable letter 2005, never approved in the US
Reference only. Not medical advice.
What can go wrong
Main adverse effects are nausea, dizziness, headache, diarrhea, and insomnia. A distinctive concern is orthostatic hypotension and syncope, especially with the first dose, prompting orthostatic testing. Contraindicated with MAOIs and other serotonergic drugs (serotonin syndrome risk), with significant hepatic impairment, and in men with certain cardiac conditions; additive hypotension with PDE5 inhibitors and CYP3A4 interactions require care. US-obtained product is typically imported or compounded and not FDA-quality-assured.
This is something to take to a clinician. The Atlas can tell you what is known about dapoxetine in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Dapoxetine (brand Priligy) is the only SSRI developed specifically as an on-demand treatment for premature ejaculation. Its pharmacokinetics are tuned for taking a single dose 1-3 hours before sex rather than chronic daily dosing: rapid absorption and a short half-life. It is approved in more than 60 countries across Europe, Asia, and Latin America but is NOT approved in the United States - Johnson & Johnson received a non-approvable letter from the FDA in 2005 and the program was never resubmitted, leaving a persistent US access gap that the telehealth market exploits.
The one SSRI designed for sex on demand - that American men still cannot legally get.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
Like all SSRIs, dapoxetine inhibits the serotonin transporter (SERT), raising synaptic 5-HT and raising the ejaculatory threshold via the spinal ejaculation reflex. What makes it suitable for on-demand use is pharmacokinetic, not mechanistic: it is absorbed fast (peak ~1 hour) and cleared fast (rapid-distribution half-life ~1.5 hours, with low plasma accumulation on repeat dosing), so a single pre-coital dose acts and then leaves the system, avoiding the steady-state exposure and chronic side-effect burden of daily antidepressant SSRIs.
- Tmax
- ~1-1.5 hr
- Distribution half-life
- ~1.5 hr
- Terminal half-life
- ~19-24 hr but low accumulation
- Metabolism
- CYP2D6, CYP3A4, FMO1
- Food effect
- Modest; can be taken with or without food
Approved nearly everywhere except where it was invented
Dapoxetine's effect size is real but modest: average IELT gains of roughly 2.5-3 fold sound large because the baseline is so short (often well under a minute), so the absolute gain may be a couple of minutes. The FDA's 2005 non-approvable letter and the manufacturer's decision not to pursue US approval are the reason American men cannot get the one drug actually designed for their condition - a regulatory and commercial gap, not a verdict that the drug fails. Imported or compounded dapoxetine sold online is unregulated for US purity and dosing.
How it is taken
Taken as needed 1 to 3 hours before anticipated sexual activity, starting at 30 mg and increasing to 60 mg if needed and tolerated, no more than once per 24 hours. First-dose orthostatic hypotension and syncope are specific concerns, so an orthostatic test and counseling to sit/lie down at prodromal symptoms are advised. It should not be combined with other serotonergic drugs, MAOIs, thioridazine, or potent CYP3A4 inhibitors, and caution applies in men on PDE5 inhibitors because both can lower blood pressure.
What people get wrong
Nothing recorded here yet. There is almost certainly something people get wrong about this one, and the blank space means we have not written it down, not that the record is clean.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
Set against the alternatives
Versus chronic off-label SSRIs (paroxetine, sertraline), dapoxetine offers comparable or slightly smaller per-event latency gains but with on-demand convenience and lower chronic exposure. Versus topical anesthetics, it is systemic (no genital numbness or partner transfer) but carries nausea/dizziness and the first-dose syncope warning. Many guidelines list it as a first-line oral option where available.
Because dapoxetine is unavailable through normal US pharmacy channels, demand is met by overseas Priligy, gray-market imports, and compounded 'climax control' products sold through telehealth - all outside FDA oversight. Where approved, it is a branded prescription product.

What this entry does not yet cover
What follows is unfinished work on our side, not a finding about Dapoxetine. A short entry is not a safe entry.
- Cost and market. We have not documented what it sells for or who sells it.
- Supply chain. We have not mapped where it is made.
- Who uses it. We have not documented the user groups.
- What could change. No forward-looking summary yet.
- Children and teenagers. We have not checked what is known about younger people.

The studies that matter
Pryor et al., two phase-III RCTs (Lancet)2006
Dapoxetine 30 mg and 60 mg roughly 2.5-3x increased average IELT vs placebo and improved control and satisfaction
Pooled phase-III analyses (e.g., McMahon)2007-2011
Consistent IELT prolongation across both lifelong and acquired PE; nausea, dizziness, and headache the main dose-related dropouts
Combination with PDE5 inhibitorsvarious
Dapoxetine plus a PDE5 inhibitor benefits men with PE and comorbid ED, with attention to additive hypotension
