Penile Prosthesis and Length
IPP with Sliding / ILEP Lengthening Techniques
Evidence grade 4 of 5, Strong. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- Penile prostheses are FDA-approved for ED; lengthening add-ons are surgical techniques
Reference only. Not medical advice.
What can go wrong
General prosthesis risks: infection, mechanical failure, erosion, and the need for revision. Lengthening add-ons add risk of glans ischemia and necrosis (rare but serious), hematoma, transient glans numbness, and temporary anorgasmia; one series reported postoperative hematoma ~20%, temporary partial glans numbness ~3.8%, temporary anorgasmia ~7%, and infection ~0.24%. These are surgical risks accepted in the context of treating ED, not cosmetic enhancement.
This is something to take to a clinician. The Atlas can tell you what is known about penile prosthesis and length in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
An inflatable penile prosthesis (IPP) is an FDA-approved, evidence-based treatment for erectile dysfunction refractory to other therapies. Because many prosthesis patients report subjective length loss, surgeons have developed length-preserving and length-enhancing add-ons - the sliding technique and its modifications (MoST), and combined incision approaches sometimes grouped under 'ILEP' (increasing length with expanders/incisions and prosthesis). Series report meaningful stretched-length gains (e.g., ~3.1 cm mean in one MoST series; ~1-3 cm typical), but these are done in the context of implanting a prosthesis for ED, carry surgical risk including the rare but feared glans ischemia/necrosis, and are not offered as cosmetic enlargement for men with normal erectile function.
A legitimate ED reconstruction - not a size procedure for normal anatomy.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
The prosthesis restores rigidity mechanically. Lengthening techniques address the shortening many patients perceive: the sliding/MoST approaches make controlled tunical incisions to allow the corpora to elongate (covering defects with fascia rather than grafts in MoST), and length/girth-expanding cylinders add modest dimension. The gain is real corporal length change, made possible because the corpora are already being opened for the implant.
Right operation, right patient
These techniques are legitimate precisely because they piggyback on a medically indicated operation (implanting an IPP for ED). The same corporal incisions would be unjustifiable as pure cosmetic enlargement in a man with normal erections, given risks like glans ischemia. Patient-perceived shortening after standard IPP often reflects perception more than measured stretched-length change, which is part of why length-preservation techniques exist.
Safety in detail
Penile prosthesis surgery and associated lengthening techniques are major procedures with risks including infection, glans ischemia/necrosis, and device complications; they require evaluation by a qualified prosthetic urologist and are indicated for erectile dysfunction, not cosmetic enlargement of normal anatomy.
What people get wrong
Often saidA penile implant is a size upgrade for anyone
ActuallyImplants are for refractory erectile dysfunction; lengthening techniques are add-ons within that reconstruction, not cosmetic procedures for men with normal erections.
Often saidLengthening at implant is risk-free extra length
ActuallyIt adds surgical risk including rare glans ischemia/necrosis; realistic added length is ~1-3 cm in the context of a medically indicated operation.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
Prosthesis surgery is a legitimate, often insurance-covered treatment for ED performed by prosthetic urologists; the ethical concern is cash-pay marketing that blurs the line between ED reconstruction and cosmetic enlargement.
Set against the alternatives
Unlike every cosmetic method in this category, prosthesis-based lengthening rests on a legitimate, FDA-approved intervention for ED and produces real corporal length change - but only appropriately within ED reconstruction. It is not a route to enlargement for men with normal erectile function, for whom the risk profile is unacceptable.
