PRP / 'P-Shot': What the Evidence Actually Shows
Platelet-Rich Plasma for ED, the Poulios RCT, the Priapus Trademark, and the Marketing Apparatus
Evidence grade 2 of 5, Thin. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- Not FDA-approved for ED; PRP kits cleared only for bone-graft/orthopedic prep
Reference only. Not medical advice.
What can go wrong
Autologous PRP is low-risk for transmissible infection (it is the patient's own blood) but injection carries bruising, pain, hematoma, and rare infection risk. The dominant harms are financial and informational: $1,500-$3,000+ per injection, often sold as a multi-session package, for an intervention with a single small positive RCT. Bundling with 'exosome' or amniotic products escalates both cost and regulatory/safety concerns.
This is something to take to a clinician. The Atlas can tell you what is known about prp / 'p-shot': what the evidence actually shows in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Platelet-rich plasma (PRP) is a concentrate of the patient's own platelets, spun from their blood, injected into the penis on the theory that platelet-derived growth factors (PDGF, VEGF, TGF-beta) drive tissue regeneration. The 'P-Shot' or 'Priapus Shot' is a TRADEMARKED branded protocol licensed to a provider network by Charles Runels, MD (who also trademarked the 'O-Shot' and 'Vampire Facial'). The single best piece of evidence is the Poulios et al. 2021 double-blind RCT, modest, statistically significant IIEF improvement vs placebo at some timepoints in mild-to-moderate ED, but small (~60 patients) and short. FDA-cleared PRP kits are cleared only for mixing with bone graft in orthopedics; injecting PRP into the penis for ED is an off-label, unapproved use.
One small positive RCT, a registered trademark, and a marketing machine that vastly outruns both.
What people take it for
These are the reasons people give for taking it. They are not effects it has been shown to have.
How it is supposed to work
Blood is drawn and centrifuged to concentrate platelets several-fold above baseline. Platelets release alpha-granule growth factors, platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), transforming growth factor-beta (TGF-beta), epidermal growth factor, which in other tissues promote angiogenesis, collagen remodeling, and cell proliferation. The regenerative hypothesis for ED is that intracavernosal PRP improves endothelial and neural function. Plausible in principle; unproven in durable human outcomes. Preparation is non-standardized, platelet concentration, leukocyte content, and activation vary hugely between clinics, so 'PRP' is not one product.
The catch: one small RCT does not validate a branded franchise
The Poulios 2021 trial (Int J Impot Res) is genuinely the strongest data, double-blind, placebo-controlled, and it showed a real but modest signal in mild-to-moderate ED. But it was small, single-center, and short-term, and it used a defined preparation, not the heterogeneous 'P-Shot' protocols sold commercially. No large multicenter RCT confirms durability. The branded 'Priapus Shot' is a marketing/licensing construct: clinics pay to use the trademark and appear in a provider directory, which manufactures an impression of clinical legitimacy that the underlying evidence (grade 2) does not support. PRP is also commonly bundled with shockwave and 'exosome' upsells, compounding unproven claims.
What people get wrong
Often saidThe P-Shot is a proven, branded medical procedure
ActuallyThe 'Priapus Shot' is a trademark licensed to clinics, not an FDA-approved therapy. The branding implies validation the evidence does not provide.
Often saidPRP regenerates erectile tissue
ActuallyGrowth-factor biology is plausible, but only one small RCT shows even a modest short-term human benefit; durable tissue regeneration in ED is unproven.
Often saidFDA-cleared kit means FDA-approved treatment
ActuallyPRP kits are cleared only for preparing PRP for bone grafting/orthopedics. Penile injection for ED is off-label and not FDA-approved.
Who profits, and who pushes back
Sexual Medicine Society of North America
PRP and similar regenerative ED therapies should be considered experimental and offered only within research/IRB settings, not marketed as proven.
Ranjith Ramasamy, MD
We need rigorous sham-controlled trials before these therapies enter routine practice; current commercial marketing has run far ahead of the science.
What we do not know
- Can a multicenter RCT replicate the Poulios signal with a standardized preparation?
- What platelet concentration, leukocyte content, and activation actually matter?
- Is any benefit durable beyond a few months?
- How much of clinical 'response' is placebo, given injection ritual and cost?
At a glance
- Best RCT
- Poulios 2021, n~60, double-blind
- Effect
- Modest IIEF gain at some timepoints
- Trademark holder
- Charles Runels, MD ('Priapus Shot')
- FDA clearance
- PRP kits: orthopedic/bone-graft only
