What can go wrong

Adulteration is the dominant safety problem: Southeast Asian 'aphrodisiac' jamu and Tongkat Ali products have repeatedly been found spiked with undeclared sildenafil/tadalafil analogues, and DNA-barcoding studies show frequent species substitution and contamination. Standardized extracts (e.g., Physta) are the studied form; raw roadside or online products carry heavy-metal and drug-adulteration risk. Tongkat Ali's testosterone effect, while real in some trials, is modest and not a substitute for evaluation of true hypogonadism.

This is something to take to a clinician. The Atlas can tell you what is known about southeast asian jamu & folk practice in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.

Jamu is Indonesia's indigenous herbal medicine tradition, prepared as fresh tonics, powders, and pills, and recognized by UNESCO in 2023 as intangible cultural heritage. Its most exported male-health ingredient is Tongkat Ali (Eurycoma longifolia, locally 'Pasak Bumi'), traditionally used across Indonesia, Malaysia, and Thailand as an aphrodisiac and energy tonic. Unlike most traditional remedies, Tongkat Ali has accumulated several randomized controlled trials suggesting modest increases in total testosterone and improvements in sexual well-being, which places its evidence above typical folk claims, though trials are small and many are industry-linked.

Jamu is a living folk pharmacy, and its star male tonic now has real (if modest) RCT data.

What people take it for

These are the reasons people give for taking it. They are not effects it has been shown to have.

  • Male libido and 'vitality' tonic
  • Mild support of total testosterone in aging/stressed men (per RCTs)
  • Fatigue, stress, and post-illness recovery (traditional)

What people get wrong

  • Often saidTongkat Ali works like TRT

ActuallyIt nudges total testosterone modestly in some men; it does not produce the large, reliable hormonal shifts of prescribed testosterone, and it is not a treatment for diagnosed hypogonadism.

  • Often saidA jamu aphrodisiac that 'works fast' is just strong herbs

ActuallyFast, drug-like potency in a herbal tonic is a red flag for sildenafil/tadalafil adulteration, which is common and can be dangerous with nitrates or heart disease.

Who profits, and who pushes back

Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.

How we got here

Jamu predates written record in the Indonesian archipelago and is depicted on the reliefs of 8th-9th century temples such as Borobudur; recipes passed largely through women ('mbok jamu' sellers) and palace traditions of Java. Across the Malay world and Thailand, Tongkat Ali's root was dug, decocted, and drunk as a stamina and virility tonic, its name ('Ali's walking stick') a frank reference to the indication. In the late 20th and 21st centuries Malaysian and Indonesian institutions standardized root extracts, and Tongkat Ali became a global supplement marketed for testosterone, while jamu itself was elevated to a point of national cultural pride and, in 2023, UNESCO recognition.

The people taking it

Jamu remains an everyday practice in Indonesia (home-brewed and street-sold) and is woven into Malay and Thai folk medicine. Tongkat Ali has globalized as a standalone supplement, heavily marketed by the men's-health and 'natural testosterone' niche in North America, Europe, and East Asia.

What we do not know

  • How large and durable is the testosterone effect in well-powered, independent trials?
  • Can third-party testing reliably exclude pharmaceutical adulteration at retail?
  • Will producer nations capture fair value as Tongkat Ali globalizes?

The market for this

What follows describes how this is bought and sold. It is not a buying guide and it is not an endorsement of any seller.

Wild Eurycoma harvesting has raised sustainability and sovereignty concerns; Malaysia in particular has sought to protect and standardize its national resource. Benefit-sharing with source communities and producer nations is uneven, and the unregulated global market is rife with mislabeled, substituted, or sildenafil-spiked product.

The studies that matter

  • RCT of standardized Eurycoma (Physta) in aging men2012

Improved quality-of-life and sexual well-being scores vs placebo in a randomized, double-blind trial

  • 6-month RCT, Tongkat Ali plus concurrent training in ADAM2021

Both exercise and Eurycoma improved erectile function; combination gave the largest gains, with testosterone increases in a subset

  • Systematic review/meta-analysis of Eurycoma trials (Leisegang et al.)2022

Reviewed nine clinical studies; pooling the five RCTs in meta-analysis found a significant increase in serum total testosterone overall, driven by and significant in the hypogonadal subgroup (not significant in eugonadal men)
