What can go wrong

Sildenafil is contraindicated with nitrates, where combined vasodilation can cause dangerous hypotension. Historically this drug-class interaction shaped ED prescribing safety standards. Common effects include headache, flushing, dyspepsia, and visual color changes. This is a historical/biographical entry; clinical use requires physician evaluation.

This is something to take to a clinician. The Atlas can tell you what is known about the discovery of viagra in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.

Sildenafil (UK-92,480) was developed at Pfizer's Sandwich, Kent laboratory in the late 1980s as a candidate treatment for angina and hypertension, targeting the PDE5 enzyme to relax blood vessels. The cardiovascular effect was modest, but trial volunteers reported a striking side effect, penile erections. Pfizer pivoted the program toward erectile dysfunction, and on March 27, 1998 the FDA approved Viagra, the first effective oral ED drug, launching the modern era of sexual medicine.

The accidental side effect that became a billion-dollar medicine.

What people take it for

What it is taken for. Approval, where it exists, covers only some of these.

  • Oral treatment for erectile dysfunction (the eventual indication)
  • Originally trialed for angina pectoris
  • Sildenafil later separately approved for pulmonary arterial hypertension (Revatio)

How it works

Sildenafil inhibits phosphodiesterase type 5 (PDE5), the enzyme that degrades cyclic GMP in penile smooth muscle. With cGMP preserved, smooth muscle stays relaxed and the corpora cavernosa fill with blood during arousal. The same vasodilatory action explains its original cardiovascular development and its later use in pulmonary hypertension.

What people get wrong

  • Often saidViagra was invented as a sex drug

ActuallyIt was developed as a heart medication for angina. The erectile effect was an unplanned trial observation that redirected the entire program.

  • Often saidViagra creates an erection on its own

ActuallyIt enables an erection in response to sexual arousal by blocking PDE5; without arousal and nitric oxide signaling it does little.

Who profits, and who pushes back

  • Leonore Tiefer

    Pushes backClinical psychologist; medicalization critic

Viagra's launch reframed ordinary variation in male sexual response as a treatable disease, fueling demand and recasting a complex relational experience as a pharmacological deficit.

The drug's marketing helped standardize a narrow performance-centered model of male sexuality and expanded the market by drawing in men without clear organic pathology.

The rapid blockbuster uptake outpaced understanding of risks such as nitrate interactions and rare non-arteritic anterior ischemic optic neuropathy, which surfaced largely after mass adoption.

How we got here

In the mid-1980s, medicinal chemists at Pfizer's Sandwich, Kent laboratory in the United Kingdom were pursuing inhibitors of phosphodiesterase type 5 (PDE5) as a way to relax vascular smooth muscle and treat angina pectoris, chest pain from reduced cardiac blood flow. By 1989 the team had synthesized the compound UK-92,480, later named sildenafil citrate. In early clinical trials the anti-anginal effect proved underwhelming, and the short half-life made it a poor cardiovascular drug. But trial participants, particularly in a study at the Morriston Hospital in Swansea, Wales, reported an unexpected and persistent side effect, erections. Pfizer scientists recognized the mechanism: PDE5 is highly concentrated in penile tissue, where blocking it preserves cyclic GMP and sustains the smooth-muscle relaxation that lets blood fill the corpora cavernosa. The company redirected the program toward erectile dysfunction, a condition that had until then been treated mainly with injections, vacuum devices, and implants. After successful efficacy trials, the FDA approved Viagra on March 27, 1998. The launch was a cultural and commercial phenomenon: Viagra became one of the fastest-selling drugs in history, destigmatized male sexual dysfunction, and effectively created the field of pharmacologic sexual medicine. Sildenafil's PDE5 mechanism was later separately developed for pulmonary arterial hypertension under the name Revatio.

Set against the alternatives

Viagra's discovery is the archetypal case of serendipitous drug repurposing, comparable to minoxidil (developed for hypertension, repurposed for hair loss) and bupropion (an antidepressant later used for smoking cessation). Unlike those, sildenafil created an entirely new mass market essentially overnight and triggered a competitive PDE5 race: Bayer/GSK's vardenafil (Levitra) and Lilly/ICOS's tadalafil (Cialis) both arrived in 2003, with tadalafil differentiating on its long 36-hour window, and Vivus's avanafil (Stendra) following in 2012 with the fastest onset. Against later mechanistically distinct options such as intracavernosal injection or penile prosthesis, the Viagra breakthrough mattered most for shifting ED treatment from invasive, clinic-bound procedures to a discreet oral pill.

What we do not know

  • Would the erectile side effect have been pursued at all under today's more rigid, indication-locked trial designs?
  • How much did Viagra's launch genuinely expand recognition of underlying vascular disease versus simply expand drug demand?
  • What is the full long-term safety profile of decades of population-scale PDE5 exposure, including any neurological signals?
  • How transferable is the serendipity-then-pivot model to current target-driven, AI-guided drug discovery pipelines?
