This is something to take to a clinician. The Atlas can tell you what is known about insulin & leptin signaling on the gonadal axis in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Metabolic signals gate testosterone at multiple points. Insulin resistance lowers SHBG (raising free fraction but reflecting metabolic dysfunction) and impairs Leydig-cell function, while hyperinsulinemia and inflammation from visceral fat suppress the axis. Leptin, secreted by fat, normally signals energy sufficiency to permit reproduction, but in obesity leptin resistance develops and high leptin directly suppresses Leydig-cell testosterone production. This is the molecular basis of the obesity-hypogonadism cycle: fat raises aromatase and leptin and lowers testosterone, and low testosterone promotes more fat.
Insulin resistance and leptin resistance both suppress the axis.
What it is invoked to explain
Claims this idea is used to support. Whether it supports them is the rest of the entry.
How it works
Visceral adiposity drives hyperinsulinemia, low-grade inflammation (TNF-alpha, IL-6), elevated leptin, and increased aromatase. These converge to suppress GnRH/LH pulsatility and impair Leydig steroidogenesis, while lowering SHBG. Improving insulin sensitivity (weight loss, exercise, sleep) reverses much of this, which is why metabolic interventions are among the most reliable natural testosterone levers.
What people get wrong
Often saidLow testosterone is the cause of belly fat, so fix T first
ActuallyCausation runs both ways, but in most men metabolic dysfunction (insulin resistance, visceral fat, leptin resistance) is the upstream driver, fixing metabolic health raises testosterone more reliably than the reverse.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
At a glance
- Insulin resistance
- Lowers SHBG; impairs Leydig function
- Leptin (high, resistant)
- Directly suppresses Leydig testosterone
