What can go wrong
Across methods: nodules, granuloma, asymmetry, migration, infection, and (with intravascular injection) skin/glans necrosis. Permanent injectables (PMMA, silicone oil) carry the highest disfigurement and necrosis risk and are the hardest to reverse. Autologous fat reabsorbs unpredictably, prompting repeat procedures. Even reversible HA can cause vascular occlusion if injected into the wrong plane.
This is something to take to a clinician. The Atlas can tell you what is known about girth augmentation methods in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Cosmetic girth augmentation spans reversible injectables to permanent surgery: hyaluronic acid (HA) filler (temporary, dissolvable), autologous fat transfer (variable reabsorption, lumping), dermal-fat grafts and acellular dermal matrix such as AlloDerm (surgical sheets/grafts), silicone sleeve implants (Penuma), and permanent injectables like PMMA and liquid silicone. The SMSNA 2024 position statement notes uniformly low-quality evidence. PMMA and unregulated 'permanent fillers' are the most dangerous: foreign-body granuloma, nodules, deformity, infection, and necrosis are documented, and PMMA for genital cosmesis is banned or restricted in many jurisdictions.
Five ways to add girth, ranked from reversible to disfiguring.
What people take it for
These are the reasons people give for taking it. They are not effects it has been shown to have.
How it is supposed to work
Each method adds subcutaneous or subdermal volume around the shaft without entering the corpora. Temporary fillers are gels the body clears; autologous fat is living tissue with a survival/resorption gamble; grafts and matrices provide a scaffold; silicone sleeves are mechanical; PMMA microspheres and liquid silicone are permanent particles that provoke chronic foreign-body reaction.
Why permanent injectables are the trap
PMMA microspheres and 'pharmaceutical-grade' liquid silicone cannot be dissolved like HA. The body walls them off as granulomas, hard, fixed, sometimes migrating lumps that can present years later, masquerade as abscesses on imaging, and require disfiguring excision. One filler series noted nodule formation in roughly 48% of patients. Because the material is permanent, a bad result is often permanent too.
Safety in detail
Permanent injectable fillers (PMMA, liquid silicone) for penile girth are dangerous and banned/restricted in many countries; they cause documented disfigurement that may be irreversible. All cosmetic girth procedures require PDD screening and frank risk disclosure.
What people get wrong
Often said'Permanent' filler is a feature
ActuallyPermanent means a complication is also permanent; PMMA and silicone granulomas can be unremovable.
Often saidFat transfer is natural so it is safe
ActuallyAutologous fat reabsorbs unpredictably and commonly lumps, often requiring revision.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
What we do not know
- Is any girth method durable and complication-free at 5 years in controlled study?
- How prevalent is black-market PMMA injection and its complication burden?
At a glance
- Most reversible
- HA filler (hyaluronidase-dissolvable)
- Most variable
- Autologous fat (reabsorption, lumping)
- Most dangerous
- PMMA / liquid silicone (permanent, granuloma)
- Evidence
- Uniformly low quality per SMSNA 2024
What this entry does not yet cover
What follows is unfinished work on our side, not a finding about Girth Augmentation Methods. A short entry is not a safe entry.
- Timeline. We have not built the dated record of research and regulatory events.
- Cost and market. We have not documented what it sells for or who sells it.
- Supply chain. We have not mapped where it is made.
- Who uses it. We have not documented the user groups.
- What could change. No forward-looking summary yet.
- How it compares. We have not set it against the alternatives.
- Children and teenagers. We have not checked what is known about younger people.
