The 50x Markup
Testosterone Cypionate Economics ($1.55/mg vs $0.027/mg ASP)
Evidence grade 4 of 5, Strong. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- Testosterone cypionate is FDA-approved (generic); pricing is market-driven
Reference only. Not medical advice.
What can go wrong
The economic risk to consumers is overpayment; the systemic concern is that high medication margin reinforces the recurring-revenue incentive to keep patients prescribed. Clinically, testosterone cypionate carries the standard TRT risk profile regardless of price.
This is something to take to a clinician. The Atlas can tell you what is known about the 50x markup in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
A KFF Health News 2024 investigation documented that injectable testosterone cypionate sold through some telehealth channels at an equivalent of about $1.55/mg, versus a Medicare average sales price (ASP) of about $0.027/mg, roughly a 50x markup on an inexpensive, long-generic molecule. The figure illustrates how much of telehealth TRT pricing is margin on a cheap drug rather than the cost of the medication itself.
The same molecule, sold for roughly 50 times its Medicare price.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
Testosterone cypionate is a long-off-patent generic; the active drug is cheap. Telehealth pricing bundles consultation, fulfillment, and recurring service into the medication price, producing a large markup over the wholesale/ASP cost of the molecule.
Where the money actually goes
At ~$0.027/mg ASP, the drug in a typical course costs only a few dollars. The ~$1.55/mg telehealth-channel equivalent means the overwhelming majority of what a subscriber pays for medication is margin, fulfillment, and service, not the cost of the testosterone.
Safety in detail
TRT requires clinician evaluation and ongoing monitoring; price does not change the medical risk profile. A low drug cost does not justify skipping monitoring.
What it does to fertility
Testosterone cypionate, like other exogenous testosterone, suppresses spermatogenesis and can impair fertility.
What has to be monitored
The bloodwork this needs, and the reason it belongs with a clinician rather than a website.
- Total/free testosterone
- Hematocrit
- PSA (age-appropriate)
- Estradiol
What people get wrong
Nothing recorded here yet. There is almost certainly something people get wrong about this one, and the blank space means we have not written it down, not that the record is clean.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
Platforms and pharmacies capture the ~50x spread between the molecule's ASP and the price charged to subscribers.
The market for this
What follows describes how this is bought and sold. It is not a buying guide and it is not an endorsement of any seller.
The active drug in a typical TRT course costs only a few dollars at Medicare ASP. The marked-up telehealth-channel equivalent means most of the medication charge is margin and service rather than drug cost.
