hCG Alongside TRT
Testicular Volume and Fertility Preservation, Dosing, Supply, and the Compounding Cliff
Evidence grade 3 of 5, Mixed. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- hCG FDA Approved (e.g., Pregnyl, Novarel); use alongside TRT for testicular maintenance is largely off-label
- WADA
- Prohibited (S2 Peptide Hormone / hCG prohibited in males)
Reference only. Not medical advice.
What can go wrong
hCG can raise estradiol (via increased testicular aromatization), cause gynecomastia, mood changes, and injection-site reactions, and requires reconstitution and refrigeration. Because most TRT-context use is off-label and supply is constrained, men sometimes turn to gray-market sources of uncertain quality.
This is something to take to a clinician. The Atlas can tell you what is known about hcg alongside trt in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Human chorionic gonadotropin (hCG) mimics LH, directly stimulating the testes to maintain intratesticular testosterone and some spermatogenesis even while exogenous testosterone suppresses the body's own LH. Men add it to TRT to preserve testicular volume and fertility. A frequently cited Baylor protocol used 500 IU every other day to maintain sperm parameters in men on testosterone; a 2020-era FDA action restricting compounding from bulk hCG, plus intermittent shortages of branded products, has made supply tighter and more expensive.
The shot that keeps the testicles online while testosterone shuts them down.
What people take it for
These are the reasons people give for taking it. They are not effects it has been shown to have.
How it is supposed to work
Exogenous testosterone suppresses pituitary LH, so the testes stop receiving their stimulus and shrink while intratesticular testosterone, essential for sperm production, collapses. hCG binds the same receptor as LH, directly driving the Leydig cells to keep producing intratesticular testosterone, sustaining testicular size and partial sperm output.
- Receptor
- LH/CG receptor on Leydig cells
- Route
- Subcutaneous or intramuscular
- Reconstitution
- Required; cold storage after mixing
Safety in detail
This intervention involves significant physiological effects and requires evaluation and monitoring by a qualified physician. Self-administration without medical oversight carries documented health risks. Gray-market hCG of unknown origin carries quality and contamination risks.
Replacement is not the same as a cycle
hCG is dosed to mimic physiological LH stimulation; excessive hCG can drive high intratesticular testosterone and estradiol. In supraphysiological/anabolic regimens, hCG is sometimes used to keep testicular function from collapsing during long-term exogenous androgen use, but this does not neutralize the broader risks of those regimens.
What it does to fertility
hCG is specifically used to mitigate TRT's fertility suppression by sustaining intratesticular testosterone; it preserves more spermatogenesis than testosterone alone, though it does not guarantee fertility and is not a substitute for andrology evaluation in men actively trying to conceive.
What has to be monitored
The bloodwork this needs, and the reason it belongs with a clinician rather than a website.
- Estradiol
- Total testosterone
- Semen analysis (when fertility is the goal)
- Hematocrit/CBC
- Testicular exam/symptom assessment
What people get wrong
Often saidTRT alone preserves fertility if you also feel fine
ActuallyExogenous testosterone suppresses intratesticular testosterone and spermatogenesis regardless of how a man feels; an adjunct like hCG is what protects testicular function.
Often saidhCG is easy to get cheaply
ActuallyPost-2020 compounding restrictions and branded shortages have made hCG more expensive and harder to source reliably.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
Set against the alternatives
Compared with SERMs (clomiphene/enclomiphene), hCG acts downstream at the testis and works even when the pituitary is suppressed by exogenous testosterone, whereas SERMs require a responsive HPG axis. hCG is the more direct fertility-preservation tool during active TRT, at the cost of injections, higher price, and supply uncertainty.
At a glance
- Acts like
- Luteinizing hormone (LH)
- Maintenance dosing (commonly cited)
- 250-500 IU subQ, 2-3x/week
- Fertility-focused dosing
- Higher, e.g. ~500-1500 IU every other day (clinician-directed)
- Branded products
- Pregnyl, Novarel (urine-derived); Ovidrel (recombinant)
What this entry does not yet cover
What follows is unfinished work on our side, not a finding about hCG Alongside TRT. A short entry is not a safe entry.
