Boswellia
Boswellia serrata / Indian Frankincense Resin Extract
Evidence grade 3 of 5, Mixed. Read how the Atlas grades evidence.
A 3 (mixed/moderate) reflects what the trial base actually supports for the best-studied use: short-term osteoarthritis symptom relief. A 2020 BMC Complementary Medicine and Therapies systematic review and meta-analysis pooled 7 RCTs and 545 patients and found Boswellia extract better than control on VAS pain (WMD -8.33), WOMAC pain (WMD -14.22), WOMAC stiffness (WMD -10.04), WOMAC function (WMD -10.75), and the Lequesne index, with a suggested minimum of about 4 weeks of use. The authors graded the underlying RCTs as medium-to-low quality with high heterogeneity, small samples, and several industry-sponsored standardized-extract trials (5-Loxin, Aflapin, Apres). That combination, a consistent direction of benefit resting on modest and partly conflicted trials, is a genuine 3, not a 4. Uses beyond osteoarthritis (ulcerative colitis, Crohn's, asthma) rest on small older single trials and sit closer to a 2.

What can go wrong
Generally well tolerated short-term, with mild GI effects; the real-world risk is that AKBA standardization varies widely between products, and theoretical interactions with anticoagulants, immunosuppressants, and CYP/P-gp substrates warrant clinician oversight. Avoid in pregnancy.
Boswellia serrata is a gum resin from a tree native to the dry hills of India, known in Ayurveda as sallaki or salai guggul and sold in the West as Indian frankincense. Its active pieces are boswellic acids, above all acetyl-11-keto-beta-boswellic acid (AKBA), which inhibit the enzyme 5-lipoxygenase and so blunt the leukotriene arm of inflammation, a route separate from how NSAIDs work on cyclooxygenase. The most credible clinical signal is short-term relief of osteoarthritis pain, stiffness, and function, seen across several small-to-moderate trials and a 2020 meta-analysis. Smaller older studies point toward benefit in ulcerative colitis, Crohn's, and asthma, but those rest on thin evidence. The practical catch is that AKBA content varies enormously between products, and much of it is poorly absorbed, so what is on the label often does not match what reaches the blood.
The 5-LOX inhibitor anti-inflammatory, distinct mechanism from NSAIDs.
What people take it for
These are the reasons people give for taking it. They are not effects it has been shown to have.
How it is supposed to work
Boswellia (Indian frankincense) is the resin exudate of the Boswellia genus trees, predominantly Boswellia serrata (Indian frankincense, the principal commercial source) and Boswellia carteri (Arabian frankincense, substantial historical religious and aromatic use), with substantial supplementary species (Boswellia sacra, Boswellia papyrifera, Boswellia rivae). The substantial pharmacology centers on a family of pentacyclic triterpenoids called boswellic acids, particularly the standardized targets β-boswellic acid, 11-keto-β-boswellic acid (KBA), and acetyl-11-keto-β-boswellic acid (AKBA, the most pharmacologically active and commercially standardized constituent). The substantial extract chemistry is complex, total boswellic acids comprise roughly 30-65% of the resin; AKBA typically 3-5% of total boswellic acids in unmodified extracts but standardized to 20-30% in concentrated commercial products (5-Loxin from Sabinsa standardized to 30% AKBA being the substantial commercial benchmark). The pharmacological mechanism profile centers on substantial 5-lipoxygenase (5-LOX) inhibition, a substantially distinct anti-inflammatory mechanism from the COX (cyclooxygenase) inhibition of NSAIDs and aspirin. The 5-LOX pathway produces leukotrienes from arachidonic acid; leukotrienes are potent inflammatory mediators particularly involved in asthma, allergic inflammation, inflammatory bowel disease, and joint inflammation. Boswellic acids (predominantly AKBA) inhibit 5-LOX through a substantially specific allosteric mechanism, substantially distinct from the NSAID COX inhibition and distinct from the leukotriene receptor antagonists (montelukast). The substantial mechanistic distinctness provides reasonable rationale for combination with NSAID therapy, substantial mechanistically complementary anti-inflammatory framework. Additional mechanisms include modulation of cathepsin G activity, modulation of complement system (particularly C3 convertase inhibition), modulation of NF-κB signaling (substantial overlap with curcumin mechanism), and modest matrix metalloproteinase inhibition relevant to cartilage degradation. The substantial Ayurvedic heritage is foundational, Boswellia serrata resin (Salai guggal in Sanskrit) has substantial documented use across multiple millennia in traditional Indian medicine for arthritis, asthma, inflammatory conditions, and digestive disorders. The substantial heritage extends beyond Ayurvedic medicine to substantial traditional Middle Eastern medicine (Boswellia sacra particularly for ceremonial and medicinal use), substantial historical religious use (frankincense in Christianity, Judaism, Islam, the substantial cultural and religious heritage), and substantial historical trade significance (the Frankincense Trail across the Arabian peninsula was substantial economic infrastructure for substantial periods). Pliny the Elder, Hippocrates, and various other classical writers documented substantial medicinal use; substantial modern pharmacological research has substantially confirmed underlying anti-inflammatory mechanisms. The substantial commercial development framework centers on AKBA standardization. 5-Loxin (Sabinsa, standardized to 30% AKBA) was the substantial foundational standardized commercial product, substantial 2008 Sengupta Arthritis Res Ther 5-Loxin osteoarthritis RCT provided foundational clinical evidence for the standardized formulation framework. Aflapin (Sabinsa, 20% AKBA enhanced with polysaccharide carrier providing approximately 25% improved bioavailability) represents substantial subsequent commercial development. Apres Flex (Laila Nutraceuticals, water-soluble AKBA) addresses the substantial water-solubility limitation of native boswellic acids. BosPure (PLT Health Solutions), Boswellin Super (Sabinsa - 70% organic acids of which 10% AKBA), and various other commercial products represent substantial market segmentation. The substantial M-07 Form-Factor flag content, different boswellia formulations are not pharmacologically equivalent, and AKBA standardization matters substantially for clinical outcomes. The substantial clinical evidence base centers predominantly on osteoarthritis and inflammatory joint conditions. Sengupta 2008 Arthritis Res Ther provided foundational evidence for 5-Loxin in osteoarthritis. Kimmatkar 2003 Phytomedicine provided earlier OA evidence. Vishal 2011 demonstrated knee OA improvement comparable to celecoxib at modest doses. Bannuru 2018 meta-analysis confirmed substantial OA evidence base. Inflammatory bowel disease evidence includes Madisch 2007 ulcerative colitis (boswellia extract H15 comparable to mesalamine in maintenance of UC remission) and Gerhardt 2001 Crohn's evidence. Asthma evidence includes Gupta 1998 Eur J Med Res. The substantial overall evidence base is more substantial than substantial broader supplement category but more modest than substantial pharmaceutical anti-inflammatory framework, reasonable adjunctive evidence base across inflammatory conditions. The substantial safety profile is generally favorable. Substantial Ayurvedic heritage and modern clinical experience suggest substantial tolerability at standard supplementation doses. Mild GI effects (nausea, mild abdominal discomfort, occasional loose stools) most common; typically resolve with adjustment. Substantial drug interaction profile is more modest than substantial supplement category equivalents, modest anticoagulant interaction (theoretical antiplatelet effects), modest CYP450 interaction (substantially less than curcumin). The substantial pregnancy safety data is limited; traditional Ayurvedic practitioners have substantial historical use frameworks but modern therapeutic supplementation generally avoided during pregnancy without specialist coordination.
How it is taken
Boswellia therapeutic dosing varies substantially by formulation and clinical application. 5-Loxin (Sabinsa standardized 30% AKBA): 100-250mg daily providing substantial AKBA dose; substantial Sengupta 2008 OA evidence base. Aflapin (Sabinsa AKBA + polysaccharide carrier enhanced bioavailability): 100-200mg daily; substantial Vishal 2011 OA evidence base with enhanced bioavailability framework. Apres Flex (Laila Nutraceuticals water-soluble AKBA): 100-300mg daily; reasonable bioavailability enhancement framework. Standard Boswellia serrata 60-65% boswellic acids: 300-800mg daily; conventional standardized extract framework. H15 extract (substantial European commercial heritage including Madisch 2007 UC evidence): 1200mg three times daily for inflammatory bowel disease applications. Combination curcumin-boswellia products (Curamin, Curamed): per combination product specification. Take with meals, boswellic acids are lipophilic and substantial fat content modestly enhances absorption. Treatment durations matter substantially, anti-inflammatory effects typically develop over 4-12 weeks of consistent supplementation; acute effects modest. AKBA-standardized formulations (5-Loxin, Aflapin) generally preferred over conventional boswellia extracts for clinical applications. Drug interaction profile is more modest than substantial supplement category equivalents, modest theoretical antiplatelet effects warrant routine anticoagulation coordination; modest CYP450 interaction; modest glucose-modulating effects warrant routine diabetes coordination. Pregnancy and lactation: limited modern therapeutic supplementation safety data; substantial Ayurvedic heritage includes pregnancy use frameworks; modern therapeutic supplementation generally avoided without specialist coordination. Pediatric use: limited modern therapeutic supplementation framework; specialist coordination essential. Surgical patients: discontinue 1 week pre-operatively given modest antiplatelet effects. The substantial Ayurvedic heritage framework provides reasonable supplementation framework for users prioritizing traditional medicine approach; modern AKBA-standardized formulations provide reasonable supplementation framework for users prioritizing standardized clinical evidence approach. The substantial mechanistic distinctness from NSAID COX inhibition provides reasonable framework for combination supplementation with NSAID therapy, substantial dual-pathway anti-inflammatory approach.

What people get wrong
Often saidAll boswellia supplements are equivalent.
ActuallySubstantially false. AKBA standardization matters substantially, 5-Loxin (30% AKBA) provides substantially different pharmacological profile than conventional boswellia extract (typically 3-5% AKBA). Specific formulation selection matters substantially for clinical outcomes. Among the M-07 Form-Factor flag examples.
Often saidBoswellia is just frankincense.
ActuallySubstantially related but distinct contexts. Frankincense traditionally refers to Boswellia sacra and Boswellia carteri (substantial religious and aromatic use); therapeutic supplementation typically Boswellia serrata (substantial Indian frankincense, substantial AKBA content). Substantial species-specific pharmacology variation. Frankincense essential oil (steam-distilled volatile compounds) has substantially distinct pharmacology from boswellic acid oral supplementation.
Often saidBoswellia is equivalent to NSAIDs for arthritis.
ActuallySubstantially different mechanisms, boswellia acts through 5-LOX inhibition; NSAIDs act through COX inhibition. Reasonable comparable effects for some OA patients (Vishal 2011 comparable to celecoxib at modest doses); reasonable adjunct to NSAID therapy with substantial mechanistic complementarity. Not substitute for established RA treatment (DMARDs, biologics).
Often saidHigher boswellia doses are always better.
ActuallySubstantial efficacy at standardized AKBA doses; minimal additional benefit at very high doses. Substantial cost-effectiveness considerations favor AKBA-standardized formulations at modest doses over conventional extracts at high doses.
Often saidBoswellia works immediately for pain.
ActuallyAnti-inflammatory effects typically develop over 4-12 weeks of consistent supplementation; acute analgesic effects modest. Substantial treatment duration considerations apply.
Often saidBoswellia and curcumin are redundant combinations.
ActuallySubstantially false, mechanistically complementary. Curcumin acts through NF-κB and COX-2; boswellia acts through 5-LOX. Substantial dual-pathway anti-inflammatory framework. Substantial commercial combination products (Curamin, Curamed) represent reasonable comprehensive anti-inflammatory framework.
Often saidFrankincense essential oil provides therapeutic boswellic acid supplementation.
ActuallySubstantially distinct preparation, essential oils contain steam-distilled volatile compounds (monoterpenes); boswellic acids are non-volatile pentacyclic triterpenoids. Essential oil contains minimal boswellic acid; substantial different pharmacology. Therapeutic boswellic acid supplementation requires oral extract formulations.
Who profits, and who pushes back
Substantial preclinical evidence vs modest human clinical translation
Boswellia has substantial mechanistic literature (substantial 5-LOX inhibition mechanism, substantial broader anti-inflammatory mechanism characterization) with substantial human clinical evidence predominantly for osteoarthritis (Sengupta 2008, Kimmatkar 2003, Vishal 2011, Bannuru 2018 meta-analysis) with substantially less-developed clinical evidence for RA, IBD (substantial Madisch 2007 framework but limited subsequent confirmatory trials), asthma (substantial Gupta 1998 framework but limited subsequent confirmatory trials), and broader applications. Reasonable position: substantial OA evidence supports OA adjunctive supplementation framework with AKBA-standardized formulations; broader application evidence base is more modest and warrants reasonable expectations alongside established medical management. Substantial mechanistic distinctness from NSAID COX inhibition provides reasonable framework for combination supplementation with NSAID therapy.

What this entry does not yet cover
What follows is unfinished work on our side, not a finding about Boswellia. A short entry is not a safe entry.
- Cost and market. We have not documented what it sells for or who sells it.
- Supply chain. We have not mapped where it is made.
- Who uses it. We have not documented the user groups.
- What could change. No forward-looking summary yet.
- How it compares. We have not set it against the alternatives.
- Children and teenagers. We have not checked what is known about younger people.
- Where we land. We have not taken a position on this yet.
