CBD (Cannabidiol)
Cannabidiol · Phytocannabinoid · FDA-Approved Epidiolex Drug + Dietary Supplement
Evidence grade 3 of 5, Mixed. Read how the Atlas grades evidence.
Evidence splits sharply by use and by product. For three rare, treatment-resistant childhood epilepsies, purified prescription cannabidiol (Epidiolex) earns grade 5: multiple placebo-controlled Phase 3 trials showed clear seizure reduction. Devinsky et al. (NEJM 2017) found convulsive seizures in Dravet syndrome fell to a median of 5.9 from 12.4 per month on 20 mg/kg/day versus little change on placebo. Thiele et al. (Lancet 2018) reported drop (atonic) seizures in Lennox-Gastaut syndrome fell a median 43.9% versus 21.8% on placebo. A 2020 tuberous sclerosis complex trial showed roughly 48% seizure reduction versus about 27% on placebo, supporting the August 2020 FDA approval. That prescription evidence is why FDA approved Epidiolex in June 2018. Over-the-counter CBD for anxiety, sleep, and pain is a different matter and lands around grade 2: reviews such as the 2024 systematic review of anxiety RCTs (Life, MDPI) describe results that are often contradictory, with wide dose variation and mostly small or open-label studies. Consumer CBD products are also frequently dosed far below the amounts used in trials, and label accuracy is inconsistent. FDA has not approved OTC CBD as a dietary supplement and does not intend to. A single blended number understates the gap, so the entry stays at 3 with the split stated openly: 5 for the specific prescription epilepsies, roughly 2 for the marketed wellness uses.

What can go wrong
Real hepatic and drug-interaction risk: dose-related liver-enzyme elevations, strong CYP450 interactions (warfarin/INR, clobazam, valproate), limited pregnancy data, inconsistent product quality, and no FDA approval as an OTC supplement.
Cannabidiol is the main non-intoxicating compound in Cannabis sativa, first isolated by Roger Adams in 1940 and structurally described by Raphael Mechoulam at the Weizmann Institute in the mid-1960s. Its strongest evidence is narrow and real: as the purified prescription drug Epidiolex, it reduces seizures in three rare childhood epilepsies (Dravet syndrome, Lennox-Gastaut syndrome, and tuberous sclerosis complex), which is why FDA approved it in 2018. The over-the-counter CBD sold for anxiety, sleep, and pain is a separate story, where the evidence is preliminary and the products are often dosed well below trial levels. Importantly, FDA has not approved OTC CBD as a dietary supplement and, as of January 2023, concluded it will not create that pathway under existing rules. Anyone considering CBD for a medical reason, or who takes other medications, should work with a qualified clinician rather than self-treat.

How it is supposed to work
Cannabidiol (CBD) is a non-psychoactive cannabinoid, one of the more than 100 cannabinoid compounds produced by Cannabis sativa (industrial hemp and broader cannabis plants). The substantial chemical structure (2-[(1R,6R)-3-methyl-6-prop-1-en-2-ylcyclohex-2-en-1-yl]-5-pentylbenzene-1,3-diol) involves a substituted cyclohexene ring linked to a substituted resorcinol, substantially distinct from psychoactive tetrahydrocannabinol (THC) which differs by a single ring closure but produces substantially different pharmacology. The substantial CBD vs THC distinction is foundational to the substantial regulatory framework, the 2018 US Farm Bill specifically distinguishes hemp-derived CBD (≤0.3% THC by dry weight) from broader cannabis products, providing substantial federal commercial framework for industrial hemp CBD. The substantial endocannabinoid system pharmacology framework involves CB1 and CB2 cannabinoid receptors plus the substantial broader endocannabinoid system (anandamide, 2-arachidonoylglycerol, FAAH, MAGL, and broader signaling). CBD has substantially low direct affinity for CB1 and CB2 receptors (in contrast to THC's substantial CB1 partial agonism producing psychoactive effects). Instead, CBD acts through substantial non-CB1/CB2 mechanisms: substantial allosteric modulation of CB1 receptor (reducing THC psychoactive effects when co-administered), substantial agonism at TRPV1 (vanilloid receptor 1) and other TRP channels, substantial 5-HT1A serotonin receptor partial agonism (anxiolytic mechanism), substantial GPR55 orphan receptor modulation, substantial PPAR-gamma activation, substantial adenosine reuptake inhibition, substantial FAAH inhibition (enhancing endogenous anandamide signaling), and substantial direct ion channel effects (sodium channels, calcium channels). The substantial pharmacological framework supports diverse applications. The substantial FDA-approved pediatric epilepsy indication (Epidiolex / cannabidiol oral solution, GW Pharmaceuticals, subsequently acquired by Jazz Pharmaceuticals) is among the cleanest examples of evidence-based cannabinoid pharmaceutical positioning. The substantial Epidiolex clinical trials in Dravet syndrome (Devinsky 2017 NEJM), Lennox-Gastaut syndrome (Thiele 2018 Lancet), and tuberous sclerosis complex (Thiele 2021 JAMA Neurol) demonstrated substantial seizure reduction at 20mg/kg/day doses. The substantial pediatric epilepsy indication is the only FDA-approved CBD use case (US); the broader consumer market's substantial anxiety, sleep, pain, inflammation, and various other positioning involves substantial mechanism plausibility but substantially preliminary clinical evidence at consumer market doses (25-50mg/day typical, substantially below Epidiolex 20mg/kg/day pharmacological range, for a 70kg adult, ~1400mg/day Epidiolex dose). The substantial bioavailability framework involves substantial route-of-administration complexity. Oral CBD has substantially poor bioavailability (~6-19% reported across studies) through substantial first-pass hepatic metabolism. Sublingual / oromucosal absorption (Epidiolex framework, similar consumer tincture positioning) provides somewhat better bioavailability (~20-30%) through partial bypass of first-pass metabolism. Inhalation (vaporization) provides substantially superior bioavailability (~30-50%) but is more associated with cannabis/recreational use than therapeutic positioning. Topical CBD provides substantial local effects without substantial systemic absorption. The substantial bioavailability variation creates substantial dose comparison challenges across consumer products and routes. The substantial regulatory framework variation is among the cleanest M-05 Regulatory whiplash examples in modern supplement category. The 2018 US Farm Bill legalized industrial hemp (≤0.3% THC) and hemp-derived CBD federally, substantial commercial market expansion through 2019-2024. However, the substantial FDA stance has continued to prohibit CBD as dietary supplement under DSHEA framework (substantial CBD-as-drug investigation history through Epidiolex IND framework). The substantial state-level framework varies substantially, some states permit CBD in foods and beverages; some prohibit; some require specific labeling. The substantial commercial framework has continued substantial expansion despite substantial FDA regulatory positioning that arguably prohibits much of the commercial activity. The substantial regulatory clarification continues to evolve through 2023-2025 with substantial FDA, FTC, and Congressional activity. The substantial WADA framework permits CBD specifically (though all other cannabinoids including THC are prohibited) for athletes, substantial 2018 WADA reclassification reflecting CBD's non-performance-enhancing classification. The substantial commercial framework has matured substantially through 2019-2024 with dozens of major brands including Charlotte's Web (substantial brand heritage through Charlotte Figi pediatric epilepsy story before Epidiolex), Lazarus Naturals (substantial accessible pricing positioning), Joy Organics, CBDistillery, NuLeaf Naturals, Bluebird Botanicals, Receptra Naturals, Medterra, FAB CBD, and many others. The substantial brand framework spans substantial product categories, tinctures (sublingual oil), capsules, gummies, topicals, beverages, pet products, and various other forms. The substantial pricing framework varies substantially from premium ($150+/month) to accessible ($20-50/month) positioning. The substantial product quality variation has been substantial, substantial ConsumerLab, Leafreport, and various third-party testing has documented substantial product labeling accuracy concerns particularly in earlier years of category development.
How it is taken
Standard CBD supplementation centers on substantially varied dose ranges depending on application. The FDA-approved Epidiolex pediatric epilepsy framework uses 10-20mg/kg/day (substantial pharmaceutical-grade doses), for a 70kg adult equivalent, ~700-1400mg/day. The substantial consumer market typical dose range (25-50mg/day) represents substantially lower-dose framework than pharmaceutical applications. Reasonable evidence-based dose framework: 25-50mg/day for general wellness applications; 25-100mg/day for anxiety, sleep, or pain applications; specialist coordination for higher therapeutic doses (300-600mg/day in Bergamaschi 2011 anxiety, Epidiolex 700-1400mg adult equivalent). Take sublingual CBD (tincture) held under tongue 60-90 seconds for optimal absorption; oral CBD (capsules, gummies) take with food containing fat for absorption improvement. The substantial bioavailability variation across routes (sublingual ~20-30%, oral ~6-19%, topical minimal systemic, inhalation ~30-50% with safety concerns) creates substantial dose comparison challenges across products. Treatment durations matter substantially, acute effects develop within 30-90 minutes for sublingual administration; chronic anxiety, sleep, and pain effects typically develop over 2-4 weeks of consistent supplementation. The substantial form selection involves substantial considerations: full spectrum (preserves entourage effect plus trace THC ≤0.3%); broad spectrum (entourage effect framework without THC); isolate (pure CBD only); topical (localized applications); pharmaceutical Epidiolex (FDA-approved pediatric epilepsy only). The substantial drug interaction profile is among the most extensive in the supplement category, CYP3A4, CYP2C9, CYP2C19, CYP2D6 substantial inhibition creates substantial interactions with statins, calcium channel blockers, immunosuppressants, anticoagulants (substantial warfarin INR variability documented), antidepressants, anticonvulsants, and many other medications. Specialist coordination essential for users on prescription medications. Pregnancy and lactation: avoid by precautionary principle given substantial FDA warning and theoretical fetal development concerns. Pediatric use outside FDA-approved Epidiolex indications: pediatric neurology coordination essential. The substantial regulatory framework variation creates substantial commercial considerations, verify state and federal regulatory compliance; substantial WADA framework permits CBD specifically for athletes (other cannabinoids including THC prohibited).

What people get wrong
Often saidCBD makes you high.
ActuallyIt does not. CBD is non-psychoactive, substantially distinct from THC which produces psychoactive effects. CBD may produce substantial relaxation through 5-HT1A and adenosine modulation but does not produce intoxication or impairment.
Often saidAll CBD products are equivalent.
ActuallySubstantial product quality variation; substantial form differentiation (full spectrum vs broad spectrum vs isolate; tincture vs capsule vs gummy vs topical). Substantial bioavailability variation across routes; substantial brand quality variation. Reputable brands with third-party Certificate of Analysis verification matter substantially.
Often saidHemp seed oil and CBD oil are the same.
ActuallyThey are not. Hemp seed oil contains minimal CBD; CBD oil contains substantial CBD extracted from hemp aerial parts (flowers, leaves, stems). Substantial commercial confusion; verify product specification.
Often saidCBD has no side effects because it's natural.
ActuallySubstantial drug interaction profile through hepatic CYP450 inhibition (CYP3A4, CYP2C9, CYP2C19, CYP2D6) creates substantial drug interaction concerns. Common side effects include somnolence, fatigue, decreased appetite, diarrhea. Specialist coordination for users on prescription medications.
Often saidHigher CBD doses are always better.
ActuallyDose-response is complex, substantial preliminary evidence supports modest effects at consumer doses (25-50mg) for general applications; substantial therapeutic doses (300-1400mg) may produce more substantial effects but with substantial cost and substantial side effect considerations. Standard supplementation should reflect intended application.
Often saidCBD cures cancer.
ActuallySubstantial preclinical cancer mechanism evidence; substantial preliminary palliative applications evidence. Not substitute for established cancer treatment. Specialist oncology coordination essential.
Often saidCBD is FDA-approved for general wellness.
ActuallyIt is not. Only Epidiolex (pediatric epilepsy: Dravet syndrome, Lennox-Gastaut syndrome, tuberous sclerosis complex) has FDA approval. Consumer wellness CBD products are not FDA-approved; FDA has not classified CBD as dietary supplement under DSHEA framework.
Who profits, and who pushes back
FDA dietary supplement framework prohibition
Despite 2018 US Farm Bill federal hemp legalization, FDA continues to prohibit CBD as dietary supplement under DSHEA framework (citing substantial Epidiolex investigational new drug history). The substantial commercial market has continued substantial expansion despite substantial FDA regulatory positioning that arguably prohibits much commercial activity. The substantial commercial framework tension creates substantial consumer purchase considerations and substantial brand regulatory compliance complexity. Reasonable position: hemp-derived CBD (≤0.3% THC) is federally legal under Farm Bill framework; substantial state-level regulatory variation; FDA regulatory positioning continues to evolve; consumers should verify current regulatory status, support reputable brands with substantial regulatory compliance positioning, and recognize the substantial regulatory framework instability.
Consumer market dose vs pharmacological dose
The substantial consumer market typical dose range (25-50mg/day) substantially differs from FDA-approved Epidiolex pediatric epilepsy framework (10-20mg/kg/day, ~700-1400mg/day adult equivalent). The substantial Bergamaschi 2011 anxiety RCT used 600mg single dose. The substantial dose disconnect raises questions about whether consumer market doses produce clinically meaningful effects versus reflecting marketing-driven dose conservatism for cost and regulatory reasons. Reasonable position: substantial preliminary clinical evidence supports modest effects at consumer doses for anxiety, sleep, and pain applications; substantial therapeutic effects may require substantially higher doses than typical consumer market positioning; specialist coordination for users seeking substantial therapeutic effects beyond consumer wellness positioning.

How we got here
CBD's biological history traces to substantial mid-20th century cannabis research with substantial Raphael Mechoulam (Hebrew University of Jerusalem) foundational characterization of cannabinoid chemistry. The 1963-1964 Mechoulam team isolation and structural characterization of CBD established the foundational chemistry distinct from psychoactive THC. The substantial decades-long Mechoulam research program established substantial endocannabinoid system pharmacology framework that continues to shape modern cannabinoid research. The substantial regulatory history involves substantial complexity. The 1937 Marihuana Tax Act and 1970 Controlled Substances Act established substantial federal cannabis prohibition framework that substantially restricted cannabinoid research and commercial development through subsequent decades. The substantial state-level medical marijuana movement through the 1990s-2010s created substantial pressure for substantial federal regulatory framework evolution. The Charlotte Figi pediatric epilepsy case (2011-2013) substantially recalibrated public perception of CBD therapeutic applications, Charlotte's Web Hemp Extract (the strain and product subsequently associated with Stanley Brothers) provided substantial seizure reduction in pediatric Dravet syndrome. The substantial Charlotte's Web brand emerged from this framework and substantial subsequent consumer market influence. The substantial CNN coverage (Sanjay Gupta documentaries) provided substantial public awareness expansion. The substantial Epidiolex pharmaceutical development era developed substantially through GW Pharmaceuticals (UK pharmaceutical company) clinical research program through the 2010s. The Devinsky 2017 NEJM Dravet syndrome trial, Thiele 2018 Lancet Lennox-Gastaut syndrome trial, and Thiele 2021 JAMA Neurology tuberous sclerosis complex trial supported the substantial FDA approval framework. FDA approval of Epidiolex (cannabidiol oral solution) for Dravet syndrome and Lennox-Gastaut syndrome in 2018 was the first FDA-approved cannabinoid-derived pharmaceutical. Jazz Pharmaceuticals subsequently acquired GW Pharma in 2021 for $7.2 billion. The substantial 2018 US Farm Bill legalized industrial hemp (≤0.3% THC by dry weight) and hemp-derived CBD federally, substantial commercial market expansion enabling substantial consumer CBD industry growth through 2019-2024. The substantial commercial market expansion created substantial regulatory framework challenges as FDA continued to prohibit CBD as dietary supplement under DSHEA framework while commercial activity proceeded substantially. The substantial consumer market expansion era developed substantially through 2019-2024 with dozens of major brands emerging (Charlotte's Web, Lazarus Naturals, Joy Organics, CBDistillery, NuLeaf, Bluebird Botanicals, Receptra Naturals, Medterra, FAB CBD, Aspen Green, and many others). Substantial product category expansion (tinctures, capsules, gummies, topicals, beverages, pet products) drove substantial mass-market retail expansion. The 2019 EVALI vitamin E acetate vape lung injury crisis substantially affected the CBD vape category specifically and broader vape category generally. The substantial third-party testing expansion era through 2020-2024 substantially improved product quality verification, ConsumerLab, Leafreport, ISO-accredited testing labs provided substantial Certificate of Analysis (COA) framework for consumer purchase decisions. The substantial commercial category matured substantially through brand consolidation, regulatory framework evolution, and quality verification expansion. The substantial 2018 WADA reclassification of CBD specifically as permitted (other cannabinoids including THC remain prohibited) reflected substantial athletic supplement framework evolution. The substantial CBD-athlete framework continues to develop through professional sports league policies and Olympic framework evolution. The modern category continues to mature through 2024-2025 with substantial commercial framework consolidation, regulatory framework continued evolution (FDA Congressional pressure for CBD framework clarification), substantial third-party testing expansion, and substantial mainstream medical integration of CBD applications. The substantial Jazz Pharmaceuticals Epidiolex pharmaceutical framework continues; the substantial consumer market framework continues substantial expansion despite substantial regulatory framework uncertainty.

What we do not know
- Whether the substantial FDA regulatory positioning (continuing to prohibit CBD as dietary supplement under DSHEA framework) will be substantially clarified through Congressional action, FDA rulemaking, or court rulings.
- Whether the substantial consumer market typical dose range (25-50mg/day) produces clinically meaningful effects versus pharmaceutical-grade dose range (700-1400mg/day Epidiolex equivalent).
- Whether the "entourage effect" (full spectrum vs isolate) produces clinically meaningful differences in head-to-head clinical trials, substantial mechanism plausibility but limited rigorous comparison evidence.
- Whether the substantial drug interaction profile (CYP3A4, CYP2C9, others) will produce sufficient clinical concern to recommend more substantial prescribing physician coordination for routine CBD users.
- Whether genetic variants affecting CBD metabolism (CYP2C19, others) produce individual variability warranting individualized dosing frameworks.
- Whether the substantial commercial category will continue mass-market retail expansion or face substantial regulatory restriction in coming years.
- Whether the substantial preliminary evidence for anxiety, sleep, and pain applications will mature into substantial mainstream medical guideline integration.
- Whether the substantial CBD-pregnancy avoidance framework should be reassessed through specific pregnancy safety studies, current avoidance is precautionary rather than based on substantial harm evidence.
- How the substantial cannabis-CBD-THC regulatory framework continues to evolve across US state and federal levels.
- Whether the substantial Epidiolex pharmaceutical positioning will expand to additional indications or remain limited to pediatric epilepsy framework.
Legal status
Substantially affected by 2018 US Farm Bill federal hemp legalization and substantial state-level regulatory framework variation. Federal commercial availability of hemp-derived CBD (≤0.3% THC by dry weight) under the 2018 Farm Bill. FDA regulatory positioning continues to prohibit CBD as dietary supplement under DSHEA framework; FDA enforcement has been substantially selective. State-level framework varies substantially. Pharmaceutical: Epidiolex (Jazz Pharmaceuticals, GW Pharma origin) is the only FDA-approved CBD pharmaceutical, prescription required for pediatric epilepsy indications (Dravet syndrome, Lennox-Gastaut syndrome, tuberous sclerosis complex). Major consumer CBD brands include Charlotte's Web (substantial brand heritage through Charlotte Figi pediatric epilepsy story before Epidiolex), Lazarus Naturals (substantial accessible pricing), Joy Organics, CBDistillery, NuLeaf Naturals, Bluebird Botanicals, Receptra Naturals, Medterra, FAB CBD, Aspen Green, R&R Medicinals, and many others. Substantial mass-market retail expansion through 2019-2024, pharmacies, supermarkets, supplement retailers, dispensaries (in cannabis-legal states), online direct-to-consumer subscription frameworks. Pricing varies substantially, premium positioning $100-200/month for therapeutic doses; accessible positioning $20-50/month for general wellness doses. Substantial subscription discount frameworks. WADA framework: CBD specifically permitted for athletes (other cannabinoids including THC prohibited), 2018 WADA reclassification reflecting non-performance-enhancing classification. Production sourcing: hemp cultivation through US (Kentucky, Oregon, Colorado, North Carolina, various other states) and international (Switzerland, Lithuania, various others); extraction through CO2, ethanol, or hydrocarbon methods; substantial supply chain transparency varies by brand. Quality verification through third-party testing (ConsumerLab, Leafreport, USP, NSF, various others) provides substantial consumer assurance. The substantial commercial category has matured substantially through 2019-2024 with substantial brand consolidation, regulatory framework evolution, and third-party testing expansion.
Status current as of July 2026. Regulatory positions change, sometimes quickly.
The market for this
What follows describes how this is bought and sold. It is not a buying guide and it is not an endorsement of any seller.
CBD commercial sourcing involves substantial hemp cultivation infrastructure across US (Kentucky, Oregon, Colorado, North Carolina, Wisconsin, Tennessee, and various other states) and international markets (Switzerland, Lithuania, Slovenia, China, various others). The substantial 2018 US Farm Bill catalyzed substantial US hemp agricultural expansion through 2019-2022; substantial commercial framework has continued through 2023-2024. Extraction methods include supercritical CO2 (substantial premium positioning, clean extraction), ethanol extraction (substantial commercial scale, residual solvent considerations), and hydrocarbon extraction (substantial efficiency, residual solvent considerations). The substantial extraction method affects final product composition, full spectrum preserves substantial non-CBD cannabinoids and terpenes; broad spectrum removes THC; isolate provides pure CBD. Quality variation across consumer products has been substantial particularly in earlier years of category development (2019-2021); substantial improvement through 2024 with substantial third-party testing expansion. Heavy metal contamination is a substantial concern given hemp's substantial soil bioaccumulation (substantial cadmium, lead, arsenic, mercury uptake from cultivation soils); reputable manufacturers with substantial heavy metal testing address this. Pesticide contamination is a substantial concern given conventional hemp cultivation practices; organic certification provides substantial assurance. Third-party testing (ConsumerLab, Leafreport, USP, NSF, ISO-accredited testing labs) provides substantial consumer assurance. The substantial commercial supply chain continues to mature through brand consolidation, regulatory framework evolution, and quality verification expansion. Production specifically for Epidiolex pharmaceutical preparation involves substantial pharmaceutical-grade manufacturing through Jazz Pharmaceuticals (GW Pharma legacy) distinct from broader consumer supplement category supply.
