Dangerous PDE5 Drug Interactions
Nitrates, alpha-blockers, and 'poppers' (amyl/alkyl nitrites) with PDE5 inhibitors
Evidence grade 5 of 5, Settled. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- Nitrate co-administration is an absolute contraindication on PDE5 inhibitor labels
Reference only. Not medical advice.
What can go wrong
Nitrate or popper co-use with any PDE5 inhibitor risks fatal hypotension; if a man on a PDE5 inhibitor develops chest pain, nitroglycerin (the standard emergency treatment) may be unsafe for a window after dosing (about 24 hours for sildenafil/vardenafil, up to 48 hours for tadalafil), complicating cardiac emergency care. Alpha-blocker co-use risks fainting from low blood pressure. The 'poppers' interaction is especially dangerous because both are often used recreationally without medical knowledge of the combination.
This is something to take to a clinician. The Atlas can tell you what is known about dangerous pde5 drug interactions in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
The most important and best-established danger of PDE5 inhibitors is their interaction with nitric-oxide donors. Nitrates (nitroglycerin, isosorbide mono-/dinitrate) and recreational alkyl-nitrite 'poppers' (amyl/isobutyl nitrite) both flood the nitric-oxide/cGMP pathway; a PDE5 inhibitor then blocks cGMP breakdown, producing additive, profound, refractory hypotension that can cause collapse, myocardial infarction, or death. This co-administration is an absolute contraindication on every PDE5 inhibitor label. Alpha-blockers (used for BPH and hypertension) are a separate, lesser interaction causing additive blood-pressure lowering and orthostatic hypotension, managed with dose separation and timing.
The single combination that turns an ED drug into a fatal hypotensive event.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
Nitric oxide drives smooth-muscle relaxation by raising cGMP. Nitrates and nitrite poppers are exogenous NO donors. PDE5 inhibitors prevent cGMP from being broken down. Given together, cGMP accumulates systemically and unopposed, causing widespread vasodilation and a blood-pressure drop that the body cannot compensate for - the effect is synergistic, not merely additive, and can be life-threatening.
Safety in detail
Combining a PDE5 inhibitor with any nitrate medication or 'popper' (amyl/alkyl nitrite) can cause severe, potentially fatal low blood pressure. This combination is absolutely contraindicated. Disclose all medications and recreational drug use to a prescriber.
- Nitrates (prescription) absolutely contraindicated
- Alkyl-nitrite 'poppers' carry the same fatal mechanism
- Alpha-blockers cause additive, manageable hypotension
- Post-dose window limits emergency nitroglycerin use
What people get wrong
Often said'Poppers aren't medication, so they're fine with Viagra'
ActuallyPoppers are nitric-oxide donors and trigger the exact same fatal hypotensive interaction as prescription nitrates. The combination has been implicated in deaths and is explicitly warned against.
Often said'I'll just take nitroglycerin if I get chest pain'
ActuallyAfter a recent PDE5 dose, nitroglycerin may be contraindicated for up to 24-48 hours, removing a frontline emergency cardiac treatment exactly when it might be needed.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
At a glance
- Absolute contraindication
- Any nitrate + PDE5 inhibitor
- 'Poppers'
- Amyl/alkyl nitrites - same fatal mechanism
- Alpha-blockers
- Additive hypotension; manage with timing/dose
- Outcome of nitrate combo
- Severe hypotension, MI, death
