Daily Low-Dose Tadalafil
Tadalafil 2.5-5 mg Once Daily for ED and BPH
Evidence grade 5 of 5, Settled. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- FDA Approved (once-daily use, 2008-2011)
Reference only. Not medical advice.
What can go wrong
Same class risks as all PDE5 inhibitors: absolutely contraindicated with nitrates (risk of severe hypotension) and used with caution with alpha-blockers. Common effects include headache, back/muscle aches (more characteristic of tadalafil), flushing, dyspepsia, and nasal congestion. Daily dosing means continuous exposure, so drug interactions and contraindications apply every day, not just before sex.
This is something to take to a clinician. The Atlas can tell you what is known about daily low-dose tadalafil in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Taken every day at 2.5-5 mg, tadalafil keeps PDE5 continuously inhibited so erections become possible at any time without dose timing, the appeal being spontaneity rather than a scheduled pill. The FDA approved Cialis for once-daily use for ED, and in October 2011 expanded the daily indication to the signs and symptoms of benign prostatic hyperplasia (BPH) and to men who have both ED and BPH. This dual benefit makes daily 5 mg tadalafil a common choice for older men with lower urinary tract symptoms plus erectile difficulty.
The two-birds option for men with both ED and an enlarged prostate.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
Tadalafil inhibits PDE5, sustaining cGMP and smooth-muscle relaxation in the penis. Its long half-life (~17.5 hours) means a small daily dose maintains steady-state inhibition around the clock. The same smooth-muscle relaxation in the prostate and bladder neck is thought to relieve BPH-related urinary symptoms.
- Steady state
- ~5 days of daily dosing
- Spontaneity
- No timing to sexual activity
- Onset (as-needed equivalent)
- 30-45 min
How it is taken
Typically started at 5 mg once daily, taken at roughly the same time each day; 2.5 mg is an option for some patients. Steady-state coverage develops over about five days. Dose adjustments are needed in renal impairment.
Safety in detail
This is a prescription drug with an absolute nitrate contraindication. It involves real cardiovascular and drug-interaction considerations and requires evaluation and monitoring by a qualified physician. Self-sourcing without medical oversight carries documented health risks.
What has to be monitored
The bloodwork this needs, and the reason it belongs with a clinician rather than a website.
- Concurrent nitrate or guanylate-cyclase-stimulator use
- Alpha-blocker / antihypertensive co-use and orthostatic symptoms
- Cardiovascular fitness for sexual activity
- Renal function for dose adjustment
What people get wrong
Often saidDaily dosing builds up a stronger erection over time
ActuallyIt provides steady readiness, not escalating potency. The benefit is spontaneity and coverage, not a bigger effect than the as-needed dose.
Often saidIt's safer because the dose is small
ActuallyThe dose is smaller per pill, but exposure is continuous, so the nitrate contraindication and interactions apply every single day.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
Set against the alternatives
Versus as-needed sildenafil or tadalafil, daily dosing trades a steady-state convenience and BPH benefit for continuous drug exposure and (often) higher cumulative cost. It is the standard PDE5 choice when a man has both ED and bothersome urinary symptoms.
