Sildenafil vs Tadalafil
Viagra vs Cialis, Consumer Head-to-Head

The written rationale for this grade is still being prepared.
- FDA
- Both FDA Approved (sildenafil 1998, tadalafil 2003)
Reference only. Not medical advice.
What can go wrong
Both share the class-wide absolute contraindication with nitrates and caution with alpha-blockers, and both can cause headache, flushing, dyspepsia, and nasal congestion. Priapism is rare but a medical emergency with either. Neither is appropriate for men unfit for sexual activity on cardiac grounds.
This is something to take to a clinician. The Atlas can tell you what is known about sildenafil vs tadalafil in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Sildenafil (Viagra) and tadalafil (Cialis) are the two most-used PDE5 inhibitors and have broadly similar efficacy; the practical difference is timing. Sildenafil acts in about 30-60 minutes, lasts roughly 4-6 hours, and is sensitive to high-fat meals. Tadalafil has a long half-life (~17.5 hours) giving a clinically useful window up to 24-36 hours (the 'weekend pill') and a daily low-dose option, and it is less affected by food. Choice is mostly lifestyle: scheduled versus spontaneous, and tolerability of each drug's side-effect profile.
Same class, different clocks.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
Both block PDE5, preserving cGMP-mediated smooth-muscle relaxation in the corpora cavernosa so arousal can produce an erection. Neither creates an erection without sexual stimulation. The differences are pharmacokinetic, not mechanistic.
Safety in detail
Both are prescription drugs with an absolute nitrate contraindication and real cardiovascular considerations. Evaluation by a qualified physician is required; sourcing from unregulated sellers risks counterfeit or contaminated product.
What people get wrong
Often saidOne is stronger than the other
ActuallyHead-to-head data show comparable efficacy. The real difference is duration and timing, not potency.
Often saidCialis is always better because it lasts longer
ActuallyLonger duration helps spontaneity but means longer exposure to side effects and interactions. Some men prefer sildenafil's shorter, predictable window.
Who profits, and who pushes back
Cochrane / systematic reviewers
Head-to-head efficacy differences between sildenafil and tadalafil are small and inconsistent; trials are often industry-sponsored and use surrogate endpoints, so 'which is better' largely reduces to timing, tolerability, and patient preference rather than a true efficacy gap.
Cardiology / pharmacology reviewers
The convenience of tadalafil's long window cuts both ways: its ~17.5-hour half-life means a nitrate interaction or hypotensive effect persists far longer than with sildenafil, which matters for men with cardiovascular disease who may need nitrates.
Evidence-based prescribing critics
Branding like the 'weekend pill' oversells a pharmacokinetic property as a lifestyle benefit; for many men an on-demand agent is equally effective and the daily-dosing option adds cost and continuous exposure without proven added benefit for episodic use.
Set against the alternatives
A meta-analysis directly comparing the two found similar efficacy and safety, with tadalafil scoring higher on sexual confidence/spontaneity measures. Sildenafil's shorter, more predictable window suits planned encounters and is cheap; tadalafil's long tail suits spontaneity and enables daily dosing (also useful for BPH). Tadalafil more often causes back/muscle aches; sildenafil more often causes transient bluish vision changes and is more food-sensitive.

What we do not know
- Do sildenafil and tadalafil differ in long-term cardiovascular or endothelial outcomes, or only in convenience?
- Can patient-level predictors (comorbidities, partner factors, anxiety) reliably guide which PDE5 inhibitor to try first?
- Does daily low-dose tadalafil produce durable improvements in erectile or endothelial function beyond its on-demand effect?
- How much of the perceived response difference between agents is placebo, expectation, or dosing-timing error rather than pharmacology?
- What is the real-world tolerability difference (headache, flushing, back pain/myalgia) at equipotent doses?
What could change
