Fat Injection / Girth Procedures
Autologous Fat Transfer, Dermal-Fat Grafts, and Fillers for Girth
Evidence grade 2 of 5, Thin. Read how the Atlas grades evidence.
The written rationale for this grade is still being prepared.
- FDA
- No FDA approval for penile girth augmentation; PMMA banned in many jurisdictions

What can go wrong
Documented complications include significant and uneven fat reabsorption requiring repeat procedures, asymmetry and contour deformity, palpable nodules and lumps, infection, skin necrosis, and, with synthetic fillers like PMMA, granuloma, migration, chronic inflammation, and disfigurement that can be very difficult to correct. Psychological harm from poor outcomes is documented.
This is something to take to a clinician. The Atlas can tell you what is known about fat injection / girth procedures in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.
Girth augmentation injects autologous fat (or places dermal-fat grafts or synthetic fillers) under the penile skin to increase circumference. Results are unstable: fat is significantly reabsorbed, often unevenly, producing asymmetry, nodularity, and disappointment. Synthetic fillers such as PMMA carry serious complication profiles and are banned in many jurisdictions. A 2019 review concluded these methods rest on scant, low-quality evidence and can damage both physical and mental health.
Girth gains that frequently reabsorb, lump, or migrate, at meaningful risk.
What people take it for
These are the reasons people give for taking it. They are not effects it has been shown to have.
How it is supposed to work
Material is placed in the loose tissue plane beneath the penile skin and outside the tunica/erectile bodies to increase apparent girth. Autologous fat lacks a reliable blood supply in this location, so much of it is reabsorbed over months; what remains may distribute unevenly. Dermal-fat grafts aim for more stable bulk but add donor-site morbidity. Synthetic fillers (PMMA, hyaluronic acid) avoid reabsorption but risk migration, granuloma, and chronic inflammation.
Safety in detail
These are invasive cosmetic procedures with significant documented complication rates and, for some fillers, banned-substance status. They require evaluation by a qualified surgeon and carry risks to physical and mental health; guidance describes the evidence base as scant and low-quality.
What people get wrong
Often saidFat injection gives permanent girth
ActuallyAutologous fat is substantially reabsorbed, often unevenly, and frequently requires repeat procedures; permanence is not reliable.
Often saidFillers are a quick, safe shortcut
ActuallySynthetic fillers, especially PMMA, carry serious complication risk and are banned in many jurisdictions.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
