Penile Girth Procedures Compared

Fat Transfer vs HA Filler vs PMMA/Bioplasty vs Penuma, Complication Profiles

Evidence grade 2 of 5, Thin. Read how the Atlas grades evidence.

The written rationale for this grade is still being prepared.

FDA
No injectable is FDA-approved for penile girth; Penuma is cleared as a device; PMMA injection is contraindicated by many societies

Reference only. Not medical advice.

This is something to take to a clinician. The Atlas can tell you what is known about penile girth procedures compared in general. It cannot tell you what is happening in your body, and the difference matters most for exactly the symptoms that bring people to this page.

Girth augmentation spans autologous fat transfer, resorbable hyaluronic-acid (HA) filler, permanent fillers such as PMMA/bioplasty, and the Penuma silicone implant. They differ sharply in durability and complications. Fat is reabsorbed unevenly and can leave nodules or oil cysts; HA is temporary and the most forgiving but needs repeat sessions; PMMA and other permanent particulate fillers carry the worst long-term profile, granuloma, disfigurement, migration, infection, and necrosis, and are discouraged by major sexual-medicine bodies. No injectable is FDA-approved for this use.

Four very different risk profiles, and one (PMMA) most experts say to avoid.

What people take it for

These are the reasons people give for taking it. They are not effects it has been shown to have.

  • Cosmetic girth (circumference) increase
  • Correction/revision of prior augmentation (specialist)

How it is supposed to work

Fat transfer harvests autologous adipose, processes it, and injects it under the penile skin, but graft survival is unpredictable, so volume retention is partial and uneven. HA fillers add a resorbable gel that the body slowly clears over months to a couple of years, allowing reversibility (hyaluronidase) but requiring maintenance. PMMA and similar permanent fillers deposit non-resorbable microspheres intended to be encapsulated by collagen; that same permanence is what makes complications so hard to treat. Penuma is a separate category, a fixed silicone implant rather than an injected material.

Why complication profiles diverge

Reversibility is the dividing line. HA can be dissolved and fat partially remodels, so their worst outcomes are usually correctable. PMMA/bioplasty cannot be cleanly removed: case series and a Nature (Int J Impotence Research, 2023) single-center review plus systematic data report granuloma, chronic inflammation, lymphedema, migration, infection, skin necrosis, and disfiguring deformity often presenting months to years later and frequently requiring extensive reconstructive surgery. Even newer poly-L-lactic-acid fillers have produced delayed foreign-body granulomas in case reports. Vendor claims that PMMA is 'safe, natural and permanent' rest largely on short-term follow-up; long-term independent data are unfavorable.

Safety in detail

All penile girth procedures are elective surgery/injection on healthy tissue and carry risk of nodules, granuloma, asymmetry, infection, migration, skin necrosis, sensory change, and the need for corrective surgery. Permanent particulate fillers (PMMA/bioplasty) are associated with severe, sometimes irreversible disfigurement and are not recommended by SMSNA/ISSM. Seek a qualified specialist and full informed consent.

What people get wrong

  • Often saidPMMA is a safe, permanent, natural girth solution

ActuallyIndependent long-term data link permanent particulate fillers to granuloma, migration, necrosis and disfigurement; many sexual-medicine bodies recommend against them.

  • Often saidFat transfer is permanent because it is your own tissue

ActuallyGraft survival is partial and unpredictable; volume is lost unevenly and can leave nodules or oil cysts.

  • Often saidFillers and implants increase erect performance

ActuallyGirth augmentation is cosmetic; it does not improve erectile rigidity or function.

Who profits, and who pushes back

Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.

At a glance

Fat transfer
Partial, uneven reabsorption; nodules/oil cysts
HA filler
Temporary (slowly resorbed); repeatable; most forgiving
PMMA/bioplasty
Permanent; worst complication profile
Penuma
Device, not injectable; girth + apparent length

What this entry does not yet cover

What follows is unfinished work on our side, not a finding about Penile Girth Procedures Compared. A short entry is not a safe entry.

  • Timeline. We have not built the dated record of research and regulatory events.
  • Cost and market. We have not documented what it sells for or who sells it.
  • Supply chain. We have not mapped where it is made.
  • Who uses it. We have not documented the user groups.
