Mazdutide
Dual GLP-1 and glucagon receptor agonist
Evidence grade 4 of 5, Strong. Read how the Atlas grades evidence.
Multiple randomized trials, including separate phase 3 obesity studies and an additional United States phase 2 trial, directly and consistently support weight reduction. Phase 3 diabetes trials also report glycemic benefit, and governmental sources record indication-specific approvals in China and Macau. The evidence remains below the highest tier because the phase 3 evidence is concentrated in Chinese adults, the located program is sponsor-dominated, a 2026 systematic review rated some weight evidence very low certainty because of heterogeneity and few trials, and mature cardiovascular and rare-event safety evidence was not located.
- FDA
- Governmental and issuer-filed sources record approval for chronic weight management in China and Macau and for glycemic control in adults with type 2 diabetes. No United States or European Union approval was established in the reviewed sources. Approval is indication-specific and jurisdiction-specific.
- WADA
- Not named in the 2026 Prohibited List or Monitoring Program. No prohibition or non-prohibition determination is inferred solely from regulatory status.

What can go wrong
Gastrointestinal adverse events are common, and one United States trial group had substantial treatment discontinuation. The available program does not establish long-term cardiovascular benefit or rare-harm incidence.
Talk to a clinician before taking this, particularly if you have a heart condition. The Atlas can tell you what has been studied about Mazdutide. It cannot tell you how it would interact with your body, your history, or anything else you are taking, and nothing on this page is a reason to start.
Mazdutide is a synthetic peptide based on oxyntomodulin that activates GLP-1 and glucagon receptors. Randomized phase 3 trials in Chinese adults and a phase 2 trial in United States adults reported substantial weight reduction. Separate phase 3 trials found glycemic benefit in type 2 diabetes. Governmental sources record approvals in China and Macau, but no United States or European Union approval was established in this review. The evidence remains geographically concentrated and sponsor-led.
Strong replicated efficacy, with jurisdiction-specific approval and important limits on generalizability.
What people take it for
What it is taken for. Approval, where it exists, covers only some of these.
How it works
GLP-1 receptor activity supports glucose-dependent insulin signaling and reduced food intake. Glucagon receptor activity can increase energy expenditure and alter liver metabolism. The clinical conclusions in this entry come from randomized human trials, not from the proposed mechanism or mouse findings.
FDA substance records classify mazdutide as a peptide. The research literature describes it as a synthetic oxyntomodulin analogue engineered to activate GLP-1 and glucagon receptors.

A consistent efficacy signal in a concentrated evidence base
The principal obesity and diabetes trials consistently favor mazdutide, and the United States phase 2 obesity trial provides a direct result outside China. The evidence is still concentrated. Every located phase 3 efficacy report was sponsor-funded and conducted in Chinese adults, while the non-Chinese program was earlier-stage. A systematic review of nine randomized trials described the evidence base as predominantly Chinese and found substantial heterogeneity in some weight outcomes.
Safety in detail
Gastrointestinal adverse events were common across trials and were more frequent than with placebo or an active comparator in several reports. Long-term cardiovascular, mortality, pregnancy, pediatric, and rare-harm evidence remains incomplete. Trial findings apply to regulated or sponsor-supplied mazdutide, not unapproved products sold under the same name.
- GLORY-1 reported gastrointestinal events as the most frequent adverse events and described most as mild to moderate. Treatment discontinuation due to adverse events was low in that trial.
- DREAMS-1 reported diarrhea, decreased appetite, and nausea as the most common adverse events. DREAMS-2 reported more gastrointestinal adverse events with mazdutide than with its active comparator.
- The United States phase 2 trial reported gastrointestinal events as the most common adverse events and the highest adverse-event discontinuation rate in its highest-exposure group.
- No completed cardiovascular outcomes trial or human safety evidence beyond the reported trial horizons was located.
Children and teenagers
An adolescent phase 3 study was listed as ongoing. No completed pediatric efficacy or safety result was located.
What people get wrong
Often saidApproval in one country makes the product approved everywhere
ActuallyApproval is jurisdiction-specific and indication-specific. No United States or European Union approval was established in the reviewed sources.
Often saidAll reported weight figures describe the same analysis
ActuallyPublished and sponsor-reported GLORY-2 estimates differ, apparently because of different estimands or analysis populations. The difference was not resolved in this pass.
Who profits, and who pushes back
Nobody is named here yet, because we have not done that work on this entry. Read that as unfinished. Somebody almost certainly sells this, and somebody almost certainly disputes it. Assume both until we have checked.
How we got here
The names mazdutide, IBI362, LY3305677, and OXM3 refer to the same dual-receptor synthetic peptide in the reviewed records. The compound is new to the Atlas peptide library and does not duplicate an existing entry.
Early randomized phase 1 and phase 2 studies reported weight and glycemic signals. GLORY-1 and GLORY-2 then reported phase 3 obesity results in Chinese adults, while DREAMS-1 and DREAMS-2 reported phase 3 type 2 diabetes results. A United States phase 2 obesity trial published in 2026 added a non-Chinese efficacy result. Governmental sources record the first chronic weight-management approval in 2025.
- 2022 to 2024Randomized early-phase studies reported weight and glycemic effects in Chinese adults.
- 2025GLORY-1 reported phase 3 obesity results, and governmental sources recorded approval for chronic weight management in China.
- 2025 to 2026Phase 3 diabetes reports, GLORY-2, and a United States phase 2 obesity report expanded the human evidence base.
The people taking it
The strongest phase 3 evidence comes from Chinese adults with obesity, overweight, or type 2 diabetes. A United States phase 2 trial supplies a direct non-Chinese obesity result, but broader multi-ethnic phase 3 replication remains limited.
What we do not know
- Whether phase 3 efficacy and safety findings generalize across more geographic and ethnic populations.
- Whether completed cardiovascular outcomes research shows benefit, neutrality, or harm.
- How safety changes with longer exposure and larger populations.
- How mazdutide compares directly with current approved therapies in completed head-to-head trials.
- Whether material sold outside regulated or clinical supply chains matches the studied peptide.
What could change
Direct comparisonA phase 3 comparison with semaglutide has published design information, but no outcome report was included in the reviewed evidence.Cardiovascular and heart failure outcomesPrograms are registered, but clinical outcome benefit has not been established.Liver, sleep, and adolescent indicationsThese programs remain investigational and do not extend the approved indications.

What this entry does not yet cover
What follows is unfinished work on our side, not a finding about Mazdutide.
- Cost and market. We have not documented what it sells for or who sells it.
- Supply chain. We have not mapped where it is made.
- How it compares. We have not set it against the alternatives.

The studies that matter
New England Journal of Medicine, GLORY-12025
In 610 Chinese adults with obesity or overweight, mean body-weight change at the primary assessment was -10.09 percent and -12.55 percent in the mazdutide groups versus +0.45 percent with placebo. The trial was sponsor-funded and geographically concentrated.
JAMA, GLORY-22026
A second randomized phase 3 trial in Chinese adults with obesity reported mean body-weight change of -16.65 percent with mazdutide versus -1.50 percent with placebo. Vomiting was frequent, and sponsor-reported estimates used a different analysis.
The Lancet Diabetes and Endocrinology, United States phase 2 obesity trial2026
Among 179 adults, mean body-weight change ranged from -7.3 percent to -18.1 percent across mazdutide groups versus -0.9 percent with placebo. The study was sponsor-funded, and one group had substantial discontinuation.
Nature, DREAMS-1 and DREAMS-22025 to 2026
Two phase 3 reports found greater glycemic and weight effects than placebo or an active comparator in Chinese adults with type 2 diabetes. Both reports included sponsor-affiliated authors.
Diabetes, Obesity and Metabolism, systematic review and meta-analysis2026
The synthesis included nine randomized trials with 2,292 participants. It described a predominantly Chinese evidence base and rated body-weight evidence in adults without diabetes as very low certainty because of heterogeneity and few trials.
